Function and Efficacy
This product is a penicillin antibiotic. The antibacterial spectrum and mechanism of action are the same as penicillin. It has a killing effect on sensitive bacteria in the reproductive period by inhibiting the synthesis of bacterial cell wall mucopeptides. This product has antibacterial activity against most Gram-positive bacteria, some Gram-negative bacteria (such as Haemophilus), spirochetes and actinomycetes, but most Staphylococcus strains (more than 90%) including Staphylococcus aureus and coagulase-negative Staphylococci can produce β-lactamase to hydrolyze and inactivate this product. The activity of this product against most sensitive strains is 2 to 5 times weaker than that of penicillin. It has no antibacterial effect on strains that produce penicillinase.
Ingredients
The main ingredient of this product is penicillin V potassium, and its chemical name is (2S, 5R, 6R)-3,3-dimethyl-7-oxo-6-(2-phenoxyacetylamino)-4-thia-1-azabicyclo[3.2.0]-heptane-2-carboxylic acid potassium salt.

Appearance
This product is light yellow soluble granules with aromatic odor and sweet taste.
Indication
Penicillin V potassium is suitable for mild and moderate infections caused by penicillin-sensitive strains, including tonsillitis, pharyngitis, scarlet fever, erysipelas, etc. caused by streptococci; bronchitis, pneumonia, otitis media, sinusitis caused by pneumococci; and skin and soft tissue infections caused by sensitive staphylococci, etc. Penicillin V potassium can also be used as a preventive drug for recurrent rheumatic fever and infective endocarditis, and can also be used for spirochetal infections.
Usage and Dosage
Oral. Adults: 125-250 mg (1-2 packets) once for streptococcal infection, 3-4 times a day, for a course of 10 days. Pneumococcal infection: 250-500 mg (2-4 packets) each time, 4 times a day, and the course of treatment is at least 2 days after the fever subsides. Staphylococcal infection, spirochete infection (Venson angina): 250-500 mg (2-4 packets) once, 3-4 times a day. To prevent recurrence of rheumatic fever: 250 mg (2 packets) once, twice a day. To prevent endocarditis, take 2g (16 packets) orally 1 hour before tooth extraction or upper respiratory tract surgery, and then take 1g (8 packets) 6 hours later (the dose is halved for children under 27 kg). Children: According to body weight, take 2.5mg-9.3mg/kg once, 6 times a day, or 3 times a day
Adverse Reactions
1. Common gastrointestinal reactions such as nausea, vomiting, upper abdominal discomfort, diarrhea and black hairy tongue. 2. Allergic reactions: rash (especially prone to occur in patients with infectious mononucleosis), urticaria and other serum sickness-like reactions, laryngeal edema, drug fever and eosinophilia. 3. Superinfection: Long-term or large-scale use of this product can cause penicillin-resistant Staphylococcus aureus, Gram-negative rods or Candida albicans infection (brown or even black tongue coating). 4. Rare hemolytic anemia, transient increase in serum aminotransferase, leukopenia, thrombocytopenia, neurotoxicity and nephrotoxicity.
Precautions
It is contraindicated for patients with positive penicillin skin test reaction, those who are allergic to this product and other penicillins, and those with infectious mononucleosis.
Special Population Medication
Precautions for children: Not clear yet. Precautions for pregnancy and lactation: 1. This product can enter the fetus through the placenta, so pregnant women should use it with caution. 2. This product can be secreted into breast milk, which may cause sensitization to infants and cause diarrhea, rash, Candida infection, etc., so lactating women should use it with caution or stop breastfeeding during medication. Precautions for the elderly: Elderly patients should adjust the dosage or medication interval according to their renal function.
Drug Interactions
1. Probenecid, aspirin, indomethacin, phenylbutazone, and sulfonamides can reduce the excretion of this product in the renal tubules, thereby increasing the blood concentration of this product, prolonging the blood elimination half-life (t1/2β), and the toxicity may also increase. 2. When this product is used in combination with allopurinol, the incidence of rash increases significantly, so the combination should be avoided. 3. This product should not be used in combination with acetaldehyde dehydrogenase inhibitors such as disulfiram. 4. When this product is used in combination with chloramphenicol for bacterial meningitis, the incidence of long-term sequelae is higher than when either is used alone. 5. This product can stimulate estrogen metabolism or reduce its enterohepatic circulation, thereby reducing the effectiveness of oral contraceptives. 6. Chloramphenicol
