Prednisone Acetate Tablets
Function and Efficacy
[Pharmacology and Toxicology] Adrenal cortical hormone drugs have anti-inflammatory, anti-allergic, anti-rheumatic and immunosuppressive effects. The mechanism of action is as follows: 1. Anti-inflammatory effect: This product can reduce and prevent the response of tissues to inflammation, thereby reducing the manifestation of inflammation. Hormones inhibit the aggregation of inflammatory cells, including macrophages and leukocytes at the site of inflammation, and inhibit phagocytosis, the release of lysosomal enzymes, and the synthesis and release of inflammatory chemical mediators. 2. Immunosuppressive effect: including preventing or inhibiting cell-mediated immune responses, delayed allergic reactions, reducing the number of T lymphocytes, monocytes, and eosinophils, reducing the binding ability of immunoglobulins to cell surface receptors, and inhibiting the synthesis and release of interleukins, thereby reducing the transformation of T lymphocytes to lymphoblasts and reducing the expansion of primary immune responses. It can reduce the passage of immune complexes through the basement membrane and reduce the concentration of complement components and immunoglobulins. [Pharmacokinetics] This product must be reduced to the 11-hydroxyl group in the liver to show pharmacological activity, and the physiological half-life is 60 minutes. The highest content in the body is in the liver, followed by plasma, cerebrospinal fluid, pleural effusion, ascites, and kidney. In the blood, most of this product is bound to plasma proteins, and free and bound metabolites are excreted in the urine, some are excreted in their original form, and a small part can be excreted through breast milk.
Ingredients
17α,21-dihydroxypregna-1,4-diene-3,11,20-trione 469-acetate.
Appearance
This product is white tablets.
Indication
It is mainly used for allergic and autoimmune inflammatory diseases. It is suitable for connective tissue diseases, systemic lupus erythematosus, severe bronchial asthma, dermatomyositis, vasculitis and other allergic diseases, acute leukemia, malignant lymphoma and other diseases suitable for other adrenal cortex hormone drugs.
Usage and Dosage
Generally, it is taken orally at a time of 5-10 mg (1-2 tablets), and 10-60 mg (2-12 tablets) per day. If necessary, the dosage can be increased or decreased as determined by the doctor. 1. For autoimmune diseases such as systemic lupus erythematosus, nephrotic syndrome, ulcerative colitis, and autoimmune hemolytic anemia, 40-60 mg can be given daily, and the dosage can be gradually reduced after the condition stabilizes. 2. For allergic diseases such as drug-induced dermatitis, urticaria, and bronchial asthma, 20-40 mg of prednisone can be given daily, and the dosage can be reduced after the symptoms are alleviated, reducing by 5 mg every 1-2 days. 3. To prevent organ transplant rejection, generally 100 mg is taken orally daily 1-2 days before surgery, and 60 mg is taken daily one week after surgery, and then the dosage is gradually reduced. 4. For the treatment of acute leukemia and malignant tumors, 60-80 mg is taken orally daily.
Adverse Reactions
Large doses of this product can easily cause diabetes, peptic ulcers and symptoms similar to Cushing's syndrome, and have a strong inhibitory effect on the hypothalamus-pituitary-adrenal axis. Concurrent infection is the main adverse reaction.
Precautions
Patients with a history of allergy to this product and adrenal cortex hormones are prohibited from using it. Patients with hypertension, thrombosis, gastric and duodenal ulcers, mental illness, electrolyte metabolism abnormalities, myocardial infarction, visceral surgery, glaucoma, etc. are generally not suitable for use. In special cases, weigh the pros and cons and pay attention to the possibility of worsening of the condition.
Special Population Medication
Precautions for children: Children should use with caution. Precautions for pregnancy and lactation: Pregnant and lactating women should use with caution. Precautions for the elderly: The use of glucocorticoids can easily cause high blood pressure, and elderly patients, especially postmenopausal women, are prone to osteoporosis.
Drug Interactions
⑴ Nonsteroidal anti-inflammatory analgesics can enhance its ulcerogenic effect. ⑵ It can enhance the hepatotoxicity of acetaminophen. ⑶ When used in combination with amphotericin B or carbonic anhydrase inhibitors, it can aggravate hypokalemia. Long-term use with carbonic anhydrase inhibitors is prone to hypocalcemia and osteoporosis. ⑷ When used in combination with protein anabolic hormones, it can increase the incidence of edema and aggravate acne. ⑸ Long-term use with anticholinergics (such as atropine) can cause increased intraocular pressure. ⑹ Tricyclic antidepressants can aggravate the mental symptoms caused by it. ⑺ When used in combination with hypoglycemic drugs such as insulin, the dose of hypoglycemic drugs should be appropriately adjusted because it can increase blood sugar in diabetic patients. ⑻ Thyroid hormones can increase its metabolic clearance rate, so when thyroid hormones or antithyroid drugs are used in combination with it, the dose of the latter should be appropriately adjusted. ⑼ When used in combination with contraceptives or estrogen preparations, it can enhance its therapeutic effects and adverse reactions. ⑽ When used in combination with cardiac glycosides, it can increase the toxicity of digitalis and the occurrence of arrhythmias. ⑾ When used in combination with potassium-excreting diuretics, it can cause severe hypokalemia and weaken the sodium-excreting and diuretic effects of diuretics due to water and sodium retention. ⑿ When used in combination with ephedrine, it can enhance its metabolic clearance. ⒀ When used in combination with immunosuppressants, it can increase the risk of infection and may induce lymphoma or other lymphoproliferative diseases. ⒁ It can increase the metabolism and excretion of isoniazid in the liver, and reduce the blood concentration and efficacy of isoniazid. ⒂ It can promote the metabolism of mexiletine in the body and reduce the blood concentration. ⒃ When used in combination with salicylates, it can reduce the concentration of plasma salicylate. ⒄ When used in combination with growth hormone, it can inhibit the latter's growth-promoting effect.
