Dexamethasone Sodium Phosphate Injection
Function and Efficacy
Adrenal cortex hormone drugs. It has anti-inflammatory, anti-allergic, anti-rheumatic and immunosuppressive effects, and its mechanism of action is: 1. Anti-inflammatory effect: This product reduces and prevents the tissue's response to inflammation, thereby reducing the manifestations of inflammation. It can inhibit the accumulation of inflammatory cells, including macrophages and leukocytes at the site of inflammation, and inhibit phagocytosis, the release of lysosomal enzymes, and the synthesis and release of inflammatory chemical mediators. 2. Immunosuppressive effect: including preventing or inhibiting cell-mediated immune responses, delayed allergic reactions, reducing the number of T lymphocytes, monocytes, and eosinophils, reducing the ability of immunoglobulins to bind to cell surface receptors, and inhibiting the synthesis and release of interleukins, thereby reducing the conversion of T lymphocytes to lymphoblasts.
Ingredients
Dexamethasone sodium phosphate. Its chemical name is: 16α-methyl-11β,17α,21-trihydroxy-9α-fluoropregnane-1,4-diene-3,20-dione-21-phosphate disodium salt.

Appearance
This product is a colorless clear liquid.
Indication
It is mainly used for allergic and autoimmune inflammatory diseases. It is mostly used for connective tissue diseases, active rheumatism, rheumatoid arthritis, lupus erythematosus, severe bronchial asthma, severe dermatitis, ulcerative colitis, acute leukemia, etc. It is also used for the comprehensive treatment of certain severe infections and poisonings, and malignant lymphomas.
Usage and Dosage
The general dose is 2-20 mg per intravenous injection; when intravenously dripping, it should be diluted with 5% glucose injection, and the drug can be repeated every 2-6 hours until the condition stabilizes, but large-dose continuous administration generally does not exceed 72 hours. It can also be used to relieve cerebral edema caused by malignant tumors. The first dose is 10 mg intravenously, followed by 4 mg intramuscularly every 6 hours. Generally, the patient can improve in 12-24 hours, and the dose is gradually reduced after 2-4 days, and the drug is stopped after 5-7 days. For brain tumors that are not suitable for surgery, the first dose can be 50 mg intravenously, and then 8 mg is repeated every 2 hours. After a few days, it is reduced to 2 mg per day, and it is given intravenously in 2-3 times. For intrathecal injection, 5 mg is injected each time, and the injection is injected once every 1-3 weeks; intra-articular injection is generally 0.8-4 mg each time, depending on the size of the joint cavity.
Adverse Reactions
There are no obvious adverse reactions when glucocorticoids are used in physiological dose replacement therapy. Adverse reactions mostly occur when pharmacological doses are used, and are closely related to the course of treatment, dosage, type of medication, usage and route of administration. Common adverse reactions are as follows: 1. Long-term use may cause the following side effects: iatrogenic Cushing's syndrome facial and body shape, weight gain, lower limb edema, purple lines, easy bleeding tendency, poor wound healing, acne, menstrual disorders, avascular necrosis of the humeral or femoral head, osteoporosis and fractures (including vertebral compression fractures, pathological fractures of long bones), muscle weakness, muscle atrophy, hypokalemia syndrome, gastrointestinal irritation (nausea, vomiting), pancreatitis, peptic ulcer or perforation, growth inhibition in children, glaucoma, cataracts, benign intracranial pressure
Precautions
Patients with a history of allergy to this product and adrenal cortical hormones are contraindicated to use this product. It should be used in special circumstances after weighing the pros and cons and paying attention to the possibility of worsening of the condition: patients with hypertension, thrombosis, gastric and duodenal ulcers, mental illness, abnormal electrolyte metabolism, myocardial infarction, visceral surgery, glaucoma, etc. are generally not suitable for use.
Special Population Medication
Precautions for children: Children should be very cautious when using adrenocortical hormones. The use of hormones can inhibit the growth and development of children. If long-term use is necessary, short-acting or medium-acting preparations should be used, and long-acting dexamethasone preparations should be avoided. Changes in intracranial pressure should be observed. Precautions for pregnancy and lactation: Pregnant women may increase the incidence of placental insufficiency, neonatal weight loss or stillbirth. Animal experiments have teratogenic effects, so the pros and cons should be weighed before use. If a nursing mother receives a large dose of the drug, she should not breastfeed to prevent the drug from being excreted through breast milk, causing adverse reactions such as infant growth inhibition and adrenal function inhibition. Precautions for the elderly: It is easy to develop hypertension and diabetes. Elderly patients, especially postmenopausal women, may aggravate osteoporosis when using it.
Drug Interactions
1. When used in patients with tuberculosis, acute bacterial or viral infection, appropriate anti-infection treatment must be given. 2. After long-term medication, the dosage should be gradually reduced before stopping. 3. Use with caution in patients with diabetes, osteoporosis, cirrhosis, poor renal function, and hypothyroidism.
