Porcine lung phospholipid injection

Function and Efficacy

Pulmonary surfactant is a mixture of phospholipids and specific proteins as the main components, distributed on the inner surface of alveoli. Its main function is to reduce lung surface tension. The property of pulmonary surfactant to reduce surface tension is essential for maintaining alveolar stability, preventing alveolar collapse at the end of exhalation, and maintaining adequate gas exchange throughout the ventilation cycle. Severe respiratory failure in premature infants caused by lack of pulmonary surfactant due to any reason is called respiratory distress syndrome (RDS) or hyaline membrane disease (HMD). RDS is the main cause of acute morbidity and mortality in premature infants, and can also cause long-term respiratory and neurological sequelae. The purpose of developing this product is to instill exogenous surfactant into the trachea to replace the lack of endogenous pulmonary surfactant. The surface activity of this product helps it to be evenly distributed in the lungs and spread along the air-liquid interface of the alveoli. The physiological and therapeutic effects of this product in treating surfactant deficiency have been confirmed in different animal models. Premature fetal rabbits delivered by caesarean section and immediately killed showed significant improvement in lung expansion after immediate use of this product. Premature newborn rabbits were intubated with 100% oxygen and given this product through endotracheal intubation. Compared with control animals, tidal volume and lung-chest compliance were significantly improved. Treatment of premature newborn rabbits with this product (maintaining a standard tidal volume of about 10ml/kg) can increase the compliance of the lung-chest system to a level similar to that of mature newborn animals. Large-scale international open and controlled clinical trials have demonstrated the therapeutic effect of Cursu on premature infants with RDS and at risk of RDS. A single dose of this product (1.25-2.5mL/kg equals 100-200mg/kg) in premature neonates showed a rapid and significant improvement in oxygenation, a decrease in the inspired oxygen concentration (FiO2), and an increase in PaO2, PaO2/FiO2 and a/APO2 ratios; mortality and the incidence of major pulmonary complications were reduced. The second or third dose of 100mg/kg can further reduce the incidence and mortality of pneumothorax.

Ingredients

Porcine Lung Alfa

Appearance

This product is a white or milky white suspension.

Indication

Guersu (porcine alfa injection) can reduce the mortality and major pulmonary complications of respiratory distress syndrome. It is used to prevent and treat respiratory distress syndrome (RDS) in premature infants.

Usage and Dosage

The recommended dose is 100-200 mg/kg body weight for the first dose, dripped into the trachea. Depending on the clinical situation, 1-2 repeated doses of 100 mg/kg body weight each time can be given, and the interval between the two doses is 12 hours. Once respiratory distress syndrome is diagnosed, this drug should be used as soon as possible.

Adverse Reactions

Pulmonary hemorrhage is a rare but sometimes fatal complication of premature infants, with a higher incidence in premature infants with less mature development. There is no evidence that the use of this product increases the risk of this event. No other adverse reactions have been reported.

Precautions

No special taboos have been found so far

Special Population Medication

Precautions for children: See [Usage and Dosage] for details. Precautions for pregnancy and lactation: No relevant information. Precautions for the elderly: No relevant information.

Drug Interactions

After intratracheal administration, the drug is mainly stored in the lungs, and its half-life in newborn rabbits is 67 hours as determined by 14C-labeled dipalmitoylphosphatidylcholine. 48 hours after administration, only trace amounts of surfactant phospholipids are present in plasma and organs other than the lungs.

Storage

It should be stored at 2-8°C, away from light.

Packaging Specification

1.5 ml: 0.12 g
