Function and Efficacy
The hydrochlorothiazide in this product is a medium-acting diuretic, which can inhibit the renal tubular reabsorption of sodium and chloride ions, promote the excretion of sodium and chloride ions by the kidneys, and produce a diuretic effect, and can lead to a decrease in blood volume and cardiac output, resulting in a decrease in blood pressure. However, it can increase the excretion of urinary potassium, which can sometimes lead to hypokalemia. The triamterene in this product is a low-efficiency diuretic, which has the effect of retaining potassium and excreting sodium. Therefore, this product has the advantages of enhanced efficacy and reduced adverse reactions. Its efficacy is significantly better than that of each single-prescription preparation, and its diuretic effect is 2 to 3 times that of each single-prescription oral preparation.
Ingredients
This product is a compound preparation, and its components are: each tablet contains 50 mg of triamterene and 25 mg of hydrochlorothiazide.

Appearance
Capsules containing light yellow powder.
Indication
Used to treat various types of edema, such as edema or ascites caused by congestive heart failure, cirrhosis and nephrotic syndrome; also used to treat mild to moderate essential hypertension.
Usage and Dosage
Oral: 1 tablet at a time, 2 times a day. When used for hypertension, 1 tablet should be given once a day as a maintenance dose after blood pressure has recovered.
Adverse Reactions
1. The most common one is hyperkalemia. 2. The less common ones are: ① Gastrointestinal reactions, such as nausea, vomiting, stomach cramps and diarrhea; ② Hyponatremia; ③ Dizziness, headache; ④ Photosensitivity. 3. The rare ones are: ① Allergies, such as rash, dyspnea; ② Blood system damage, such as granulocytopenia or even agranulocytosis, thrombocytopenic purpura, megaloblastic anemia (interference with folic acid metabolism); ③ Kidney stones. It has been reported that the incidence of kidney stones in long-term users of this drug is 1/1500. The mechanism may be that the concentration of this drug and its metabolites in urine is supersaturated, crystallizes and combines with protein matrix, thus forming kidney stones.
Precautions
Contraindicated in case of hyperkalemia.
Drug Interactions
1. Adrenocortical hormones, especially those with strong mineralocorticoid effects, adrenocorticotropic hormone can weaken the diuretic effect of this drug and antagonize the potassium retention effect of this drug. 2. Estrogen can cause water and sodium retention, thereby weakening the diuretic effect of this drug. 3. Non-steroidal anti-inflammatory analgesics, especially indomethacin, can reduce the diuretic effect of this drug, and the renal toxicity increases when used in combination. 4. Sympathomimetic drugs reduce the antihypertensive effect of this drug. 5. Dopamine enhances the diuretic effect of this drug. 6. When used in combination with drugs that cause a decrease in blood pressure, both the diuretic and antihypertensive effects are enhanced. 7. When used in combination with the following drugs, the chance of hyperkalemia increases, such as potassium-containing drugs, stored blood (containing 30mmol/L potassium, and the potassium content of blood stored for more than 10 days is as high as 65mmol/L), angiotensin converting enzyme inhibitors, angiotensin II receptor antagonists and cyclosporine A. 8. When used in combination with glucose insulin solution, alkaline agents, and sodium-type potassium-lowering exchange resins, the chance of hyperkalemia decreases. 9 This drug prolongs the half-life of digoxin. 10 Metabolic acidosis is likely to occur when used in combination with ammonium chloride. 11 When used in combination with nephrotoxic drugs, nephrotoxicity increases. 12 Sodium carbenoxolone and licorice preparations have aldosterone-like effects and can reduce the diuretic effect of this drug. 13 Because it can increase blood uric acid, it can further increase blood uric acid when used in combination with thiazide and loop diuretics, so it should be used in combination with drugs for the treatment of gout. 14 It can increase blood sugar. When used in combination with hypoglycemic drugs, the latter's dose should be appropriately increased.
