Function and Efficacy
This product is a tetracyclic antidepressant. Its main function is to selectively inhibit the reuptake of norepinephrine in the peripheral and central nervous systems, while having no effect on the reuptake of 5-hydroxytryptamine. Due to the reduction of norepinephrine reuptake, the concentration of norepinephrine in the synaptic cleft increases, which downregulates the presynaptic membrane ?2 receptors and strengthens the postsynaptic membrane ?1 receptors, producing an antidepressant effect. This product also has an anti-anxiety effect, and its sedative, anticholinergic and blood pressure lowering effects are relatively mild.

Appearance
This product is white or off-white tablets.
Indication
It is used for various types of depression. It is also effective for post-schizophrenia depression.
Usage and Dosage
Oral. Common adult dosage: 25 mg once, 2-3 times a day, increase 25-50 mg every other day according to the condition. The effective therapeutic dose is generally 75-200 mg a day, and the maximum dose does not exceed 225 mg a day. Be careful of adverse reactions. The maintenance dose is 50-150 mg a day, taken orally in 1-2 times.
Adverse Reactions
Common anticholinergic symptoms include dry mouth, constipation, dysuria, dizziness, blurred vision and tachycardia, which are mild and often occur in the early stage of medication. Adverse reactions of the central nervous system may cause drowsiness, insomnia or agitation, and early medication may increase the risk of suicide in patients. Other symptoms include rash, postural hypotension and abnormal electrocardiogram changes, mainly conduction block. Epileptic seizures and toxic liver damage are occasionally seen.
Precautions
Epilepsy, glaucoma, urinary retention, recent history of myocardial infarction, and those who are allergic to this product.
Special Population Medication
Precautions for children: Use in children and adolescents (under 18 years of age): The safety and efficacy of Ludiomil in children and adolescents have not been established. Therefore, it is not recommended for use in this age group. Precautions for pregnancy and lactation: Animal studies on pregnancy have shown that Ludiomil has no teratogenic or mutagenic effects, and there is no evidence of impaired fertility or harm to the fetus. However, it has not been determined to be safe for use during pregnancy. Individual cases have shown that the use of Ludiomil may be associated with adverse reactions to the human fetus. Ludiomil should not be used during pregnancy unless the benefits clearly outweigh the risks to the fetus. If the patient's clinical condition permits, Ludiomil should be discontinued at least 7 weeks before the expected date of delivery to prevent possible symptoms such as dyspnea, lethargy, irritability, tachycardia, hypotonia, convulsions, tremors and hypothermia in the newborn. Lactation Maprotiline can be excreted in breast milk. After taking 150 mg orally daily for 5 consecutive days, the concentration in breast milk can reach 1.3 to 1.5 times the plasma concentration. Although reports indicate that the drug has no adverse effects on infants, mothers treated with Ludiomil should not breastfeed. Elderly precautions: (Age over 60 years old) It is advisable to gradually increase the dose. The initial dose is 10 mg each time, 3 times a day; or 25 mg, once a day; if necessary, the daily dose can be gradually increased to 25 mg, 3 times a day; or 75 mg, once a day, depending on the patient's response.
Drug Interactions
1. This product can enhance the anticholinergic effect when used in combination with antihistamines. 2. Cimetidine can increase the blood concentration of this product. 3. This product can weaken the antihypertensive effect of clonidine and guanethidine when used in combination with this product. 4. This product can increase arrhythmia when used in combination with thyroid hormones. 5. This product can increase the blood concentration of fluoxetine when used in combination with this product. It is not suitable to use it together.
