Function and Efficacy
Andytai is composed of piperacillin sodium and tazobactam sodium in a ratio of 8:1. Piperacillin sodium mainly inhibits the synthesis of bacterial cell wall mucopeptides; tazobactam sodium is an irreversible competitive beta-lactamase inhibitor that can firmly bind to the enzyme and inactivate the enzyme, thereby protecting the beta-lactam ring of piperacillin sodium and maintaining its efficacy.

Appearance
This product is white powder or off-white loose substance or powder, odorless, bitter, and extremely hygroscopic.
Indication
This product is suitable for the following moderate to severe infections caused by B-lactamase-producing bacteria that are resistant to piperacillin sodium but sensitive to piperacillin/tazobactam: 1. Appendicitis (accompanied by perforation or abscess) and appendicitis caused by piperacillin- and B-lactamase-resistant Escherichia coli and Bacteroides (Bacteroides fragilis, Bacteroides ovatus, Bacteroides polyspermum or Bacteroides vulgaris). 2. Skin and soft tissue infections caused by piperacillin- and B-lactamase-resistant Staphylococcus aureus, including cellulitis, skin abscesses, ischemic or diabetic foot infections. 3. Postpartum endometritis or pelvic inflammatory disease caused by piperacillin- and B-lactamase-resistant Escherichia coli. 4. Community-acquired pneumonia (moderate only) caused by piperacillin- and B-lactamase-resistant Haemophilus influenzae. 5.
Usage and Dosage
Dissolve an appropriate amount of this product in 20 ml of diluent (0.9% sodium chloride injection or sterile water for injection), then immediately add it to 250 ml of liquid (5% glucose injection or 0.9% sodium chloride injection) and drip it intravenously for at least 30 minutes each time. The course of treatment is 7-10 days. The course of treatment for hospital-acquired pneumonia is 7-14 days. It can be adjusted according to the condition and bacteriological examination results. For patients with normal renal function (creatinine clearance ≥90 ml/min), adults and children over 12 years old should drip 3.375 grams (containing 3 grams of piperacillin sodium and 0.375 grams of tazobactam sodium) intravenously once every 6 hours. When treating nosocomial pneumonia, the starting amount is 3.375 grams each time, once every 4 hours, and aminoglycosides are used in combination. If Pseudomonas aeruginosa is not isolated, aminoglycosides can be considered to be discontinued according to the degree of infection and condition.
Adverse Reactions
1. Common adverse reactions of this product are as follows, which mostly occur when this product is used in combination with aminoglycosides: 1. Skin reactions: such as rash, itching, etc. 2. Gastrointestinal reactions: such as diarrhea, nausea, vomiting, etc. 3. Allergic reactions. 4. Local reactions: such as local irritation reactions after injection, pain, phlebitis, thrombophlebitis and edema, etc. 5. Other reactions: such as thrombocytopenia, pancreatitis, fever, fever with eosinophilia, diarrhea or elevated transaminase, etc. 2. In addition, the following adverse reactions can also be seen with this product: 1. Diarrhea, constipation, nausea, vomiting, abdominal pain, indigestion, etc. 2. Maculopapular rash, herpes, urticaria, eczema, itching, etc. 3. Irritability, dizziness, anxiety, etc. 4. Other reactions: such as rhinitis, dyspnea, etc.
Precautions
It is contraindicated in patients who are allergic to penicillins, cephalosporins or beta-lactamase inhibitors.
Drug Interactions
1 Aminoglycosides: (1) In in vitro tests, this product can inactivate aminoglycosides when used together with them. (2) When this product is used together with tobramycin, the area under the curve, renal clearance and urinary excretion of tobramycin will decrease by 11%, 32% and 38% respectively, because piperacillin/tazobactam may inactivate tobramycin. In patients with severe renal insufficiency, such as hemodialysis patients, the pharmacokinetics of tobramycin will change when it is used in combination with piperacillin. 2 Probenecid: When this product is used in combination with probenecid, the half-life of piperacillin can be extended by 21% and the half-life of tazobactam can be extended by 71%. 3 Vancomycin: When this product is used in combination with vancomycin, the pharmacokinetics are not affected.
