Glipizide Tablets

Function and Efficacy

This product is a second-generation sulfonylurea antidiabetic drug. It is effective for most patients with type 2 diabetes, and can reduce fasting and postprandial blood sugar, and reduce glycosylated hemoglobin (HbAlc) by 1% to 2%. The main function of this type of drug is to stimulate pancreatic islet cells to secrete insulin, but the prerequisite is that pancreatic islet cells still have a certain function of synthesizing and secreting insulin. Its mechanism is to specifically bind to the sulfonylurea receptor on the cell membrane, thereby closing the K channel, causing changes in membrane potential, opening the Ca2 channel, and increasing Ca2 in the cytosol, which promotes insulin secretion. In addition, there are extra-pancreatic effects, including improving the insulin resistance of peripheral tissues (such as liver, muscle, and fat).

The main function of this type of drug is to stimulate pancreatic cells to secrete insulin, but the prerequisite is that pancreatic β cells still have a certain function of synthesizing and secreting insulin. Its mechanism is to specifically bind to the sulfonylurea receptors on the β cell membrane, thereby closing the K channel, causing changes in membrane potential, opening the Ca2 channel, and increasing intracellular Ca2, which promotes insulin secretion. Appearance

This product is a white tablet that disintegrates rapidly in water.

Indication

It is suitable for patients with mild to moderate type 2 diabetes who have not achieved satisfactory results with diet control and physical exercise for 2 to 3 months. The pancreatic islet cells of such diabetic patients must have a certain insulin secretion function, and have no acute complications (such as infection, trauma, ketoacidosis, hyperosmolar coma, etc.), no pregnancy, and no serious chronic complications.

Usage and Dosage

The oral dosage varies from person to person. The generally recommended dosage is 2.5 to 20 mg/day, taken 30 minutes before breakfast. If the daily dosage exceeds 15 mg, it is advisable to take it three times before meals in the morning, noon and evening. For those who have failed diet therapy alone: the starting dose is 2.5 to 5 mg per day, and the dosage is increased or decreased according to the blood sugar and urine sugar situation, increasing or decreasing by 2.5 to 5.0 mg each time. If the daily dosage exceeds 15 mg, take it 2 to 3 times before meals. For those who have used other oral sulfonylurea hypoglycemic drugs: stop taking other sulfonylurea drugs for 3 days, and start taking this product after rechecking blood sugar. Gradually increase the dosage from 5 mg until the ideal therapeutic effect is achieved. The maximum daily dose does not exceed 30 mg.

Adverse Reactions

1. Common symptoms include gastrointestinal symptoms (such as nausea, upper abdominal distension), headache, etc., which can be relieved by reducing the dose. 2. Some patients may experience skin allergies. 3. Hypoglycemia is occasionally seen, especially in the elderly and frail, those who are overactive, those who eat irregularly, drink alcohol, or have liver damage. 4. Reversible changes in the hematopoietic system have also been reported occasionally.

Precautions

1. Patients allergic to sulfonamides. 2. Patients with type 1 diabetes who have been diagnosed. 3. Patients with type 2 diabetes accompanied by stressful conditions such as ketoacidosis, coma, severe burns, infection, trauma, and major surgery. 4. Patients with impaired liver and kidney function. 5. Patients with leukopenia.

Special Population Medication

Precautions for children: Not tested. Precautions for pregnancy and lactation: 1. Animal experiments and clinical observations have shown that sulfonylurea hypoglycemic drugs can cause stillbirth and fetal malformation, so they are contraindicated for pregnant women. 2. This type of drug can be excreted through breast milk, so it is not suitable for nursing mothers to use it to avoid hypoglycemia in infants. Precautions for the elderly: Start with a small dose and gradually adjust the dose.

Drug Interactions

1. This drug can increase its hypoglycemic effect when used in combination with coumarins, monoamine oxidase inhibitors, phenylbutazone, sulfonamides, chloramphenicol, cyclophosphamide, probenecid, and salicylic acid drugs. 2. When used in combination with adrenaline, adrenocortical hormones, oral contraceptives, and thiazide diuretics, its hypoglycemic effect can be reduced. 3. Be cautious when using it in combination with blocking drugs. 4. Gastrointestinal diseases that shorten the retention time of this product in the gastrointestinal tract can affect the pharmacokinetics and efficacy of this product.
