Function and Efficacy

1. This product is an anti-tuberculosis drug, a compound preparation of rifampicin and isoniazid. Rifampicin has a significant bactericidal effect on Mycobacterium tuberculosis and some non-tuberculous mycobacteria (including Mycobacterium leprae, etc.) both inside and outside the host cells. Isoniazid has a highly selective bactericidal effect on all types of Mycobacterium tuberculosis, with a strong effect on Mycobacterium tuberculosis in the growth and reproduction period, and a weaker and slower effect on the dormant period. The combination of the two can enhance the antibacterial activity and reduce the production of drug-resistant strains. 2. Rifampicin firmly binds to the β subunit of DNA-dependent RNA polymerase, inhibits the synthesis of bacterial RNA, prevents the enzyme from connecting to DNA, thereby blocking the RNA transcription process and stopping the synthesis of DNA and protein. The mechanism of action of isoniazid may be to inhibit the synthesis of mycolic acid in sensitive bacteria and cause the cell wall to rupture.

Indication

It is suitable for the 4-month maintenance treatment of patients with tuberculosis who are first-time patients and non-multi-drug-resistant tuberculosis.

Usage and Dosage

The usual oral dose for adults is 3 tablets at a time for those weighing 50kg (rifampicin 0.45g, isoniazid 0.225g); 4 tablets at a time for those weighing ≥50kg (rifampicin 0.6g, isoniazid 0.3g), once a day, 30 minutes before meals or 2 hours after meals. The general course of treatment is 4 months.

Adverse Reactions

1. Digestive tract reactions are the most common. Gastrointestinal reactions such as loss of appetite, nausea, vomiting, upper abdominal discomfort, diarrhea, etc. may occur after oral administration of this product. 2. Hepatotoxicity is the main adverse reaction of this product. In the first few weeks of treatment, a small number of patients may experience elevated serum aminotransferase, hepatomegaly and jaundice. Most of them are asymptomatic transient elevations of serum aminotransferases, which can recover on their own during the course of treatment. It is more likely to occur in the elderly, alcoholics, malnourished people, those with pre-existing liver disease or other factors that cause abnormal liver function. Toxic reactions manifest as poor appetite, abnormal fatigue or weakness, nausea or vomiting (prodromal symptoms of hepatotoxicity) and dark urine, yellow eyes or skin (hepatotoxicity). 3. Precautions

1. It is contraindicated for patients who are allergic to isoniazid, rifampicin and rifamycin antibiotics. 2. It is contraindicated for patients with severe liver dysfunction, biliary obstruction and pregnant women within 3 months.

Special Population Medication

Precautions for children: The safety of this product in children under 5 years old has not been determined. Precautions for pregnancy and lactation: 1. Both rifampicin and isoniazid can pass through the placenta, and animal experiments have confirmed that they can cause teratogenesis and stillbirth. Although no problems have been confirmed in humans, pregnant women should avoid using them. If there is an indication for use, the pros and cons must be fully weighed. 2. Both rifampicin and isoniazid can be secreted through breast milk. Although no problems have been confirmed in humans, the pros and cons of using them during lactation should still be fully weighed. If the drug is used, breastfeeding should be suspended. Precautions for the elderly: Elderly patients have decreased liver function, and the dosage should be reduced as appropriate.

Drug Interactions

1. Drinking alcohol can increase the incidence of hepatotoxicity of this product and increase the metabolism of this product. The dosage needs to be adjusted, and the patient should be closely observed for hepatotoxicity. 2. Para-aminosalicylate can affect the absorption of this product, resulting in a decrease in the blood concentration of rifampicin; if combined use is necessary, the interval between taking the two should be at least 6 hours. 3. The combination of this product with ethionamide, pyrazinamide or other anti-tuberculosis drugs can aggravate its adverse reactions. Combination with other hepatotoxic drugs can increase the hepatotoxicity of this product and should be avoided as much as possible. 4. Chlorphenazine can reduce the absorption of rifampicin, delay the peak time and prolong the half-life. 5. Combination with miconazole or ketoconazole can reduce the blood concentrations of the latter two, so this product should not be used in combination with imidazoles. 6.
