Desmopressin Acetate Tablets
Function and Efficacy
The desmopressin contained in this product is similar in structure to the natural hormone arginine vasopressin. It differs from arginine vasopressin mainly in the deamination of cysteine and the substitution of D-arginine for L-arginine. These structural changes prolong the duration of action of clinical doses of desmopressin without producing the side effect of pressor. Intravenous injection of desmopressin acetate at 0.3 micrograms per kilogram of body weight increases the activity of coagulation factor VIII (VIII:C) in plasma by 2-4 times; this dose also increases von Willebrand antigen factor (vWF:Ag) and releases tissue plasminogen activator (t-PA). The administration of this product can shorten or normalize the bleeding time of patients with uremia, cirrhosis, congenital or drug-induced platelet dysfunction, and prolonged bleeding time caused by unknown causes. The use of this product can avoid the risk of human immunodeficiency virus (HIV) and hepatitis virus infection caused by the use of factor VIII preparations.

Appearance
This product is white tablets.
Indication
1. Mainly used for the treatment of central diabetes insipidus and transient diabetes insipidus caused by cranial trauma or surgery; 2. Used for the diagnosis and differential diagnosis of diabetes insipidus; 3. Treat nocturnal enuresis (patients 6 years old or older); 4. Used for renal concentrating function tests; 5. Treat hemophilia A (FⅧ:C deficiency) and von Willebrand disease (vWD); 6. Used for thrombocytopenia (foreign data).
Usage and Dosage
The dosage varies from person to person and should be adjusted accordingly. Treatment of central diabetes insipidus: The initial appropriate dose for adults and children is 0.1 mg each time, three times a day. The dose is then adjusted based on the patient's efficacy. According to clinical experience, the total daily amount is between 0.2-1.2 mg. The appropriate dose for most patients is 0.1-0.2 mg each time, three times a day. Treatment of nocturnal enuresis: The initial appropriate dose is 0.2 mg taken before bedtime. If the effect is not significant, it can be increased to 0.4 mg. After three months of continuous use, the drug should be discontinued for at least one week to assess whether continued treatment is needed. Drinking water should be limited during treatment. Adverse Reactions
Common (>1/100) side effects General reactions: Headache; Digestive system: Stomach pain and nausea; Upper respiratory tract: Nose bleeding. If desmopressin acetate is used without restricting drinking water, it may cause water retention/hyponatremia and its complications (headache, nausea/vomiting, decreased serum sodium and weight gain, and more severe cases may cause convulsions). Marketing experience: There are rare cases of emotional disorders reported in children; there are cases of skin allergies and more serious systemic allergic reactions reported.
Precautions
Patients with habitual or psychogenic thirst; patients with heart failure or other diseases who need to take diuretics.
Special Population Medication
Precautions for children: Desmopressin acetate tablets should be used with caution in young patients. Precautions for pregnancy and lactation: When pregnant women were given desmopressin acetate in doses exceeding 100 times the human dose, no cases of teratogenicity were observed; one researcher reported three cases of malformed children whose mothers used desmopressin acetate during pregnancy for diabetes insipidus, but other published case reports of more than 120 cases showed that pregnant women who used desmopressin acetate during pregnancy did not give birth to malformed children; at the same time, a review citing a large amount of data showed that 29 children whose mothers used desmopressin acetate throughout pregnancy did not develop teratogenicity. After breastfeeding women were given high doses of desmopressin acetate (300 micrograms) intranasally, breast milk was tested, which proved that the content of desmopressin acetate in breast milk was far below the dose required to affect diuresis. Precautions for the elderly: Desmopressin acetate tablets should be used with caution in elderly patients.
Drug Interactions
Some drugs that cause the release of antidiuretic hormones, such as tricyclic antidepressants, chlorpromazine, carbamazepine, etc., can increase the risk of antidiuresis and water retention. Indomethacin (Indomethacin) can increase the urine concentrating effect of desmopressin acetate, but it does not affect the duration of its effect. This effect is probably not of any clinical significance.
Storage
It is best to store at room temperature (no more than 25°C) and in a dry place (relative humidity no more than 60%). In places with high temperature or high humidity, the medicine bottle should be stored in a refrigerator. The bottle cap must be tightened (with moisture-proof agent inside).
