Vincamine
Function and Efficacy
Vincamine can increase cerebral blood flow, increase the uptake of oxygen and sugar by brain tissue, and reduce lactic acid formation. The mechanism is not yet clear. Clinical studies suggest that vincamine can counteract the degenerative process through cytoprotective effects (calcium antagonism and metabolic activation). Pharmacological studies have confirmed that vincamine can correct mental confusion and promote wakefulness by reducing the alpha rhythm and increasing the delta and theta components. The effect of vincamine is mainly on metabolism, but it can also inhibit platelet aggregation and improve blood rheological properties, thus having a good effect on microcirculation. Vincamine can effectively increase cerebral blood flow and reduce left-right hemisphere functional asymmetry in patients with mild and moderate dementia. During medication, the ischemic area can be reduced, memory can be improved, and behavior can be improved. Since no changes in arterial oxygen partial pressure or blood pressure are caused, it is speculated that vincamine may act as a vasodilator.
Indication
It is used for various diseases that require increased oxygen supply to the brain, such as cerebral arteriosclerosis, premature senile brain degeneration, Alzheimer's disease, cerebral embolism, cerebral thrombosis and hemorrhagic sequelae. For cerebral arteriosclerosis, it can eliminate its pathological changes, improve memory and general condition. It can also be used to treat mental disorders, inner ear blood flow disorders, improve tinnitus, vertigo and other symptoms, treat retinal circulation disorders, acute retinopathy and middle retinal artery embolism. Usage and Dosage
1. Oral administration: 5-20 mg each time, 2-3 times a day. Sustained-release tablets: 60 mg per day, once in the morning and evening. In severe cases, the dose can be increased to 90 mg per day. After 3-6 weeks of effectiveness, change to maintenance dose: 30 mg per day. 2. Intramuscular injection: 5-15 mg each time, 2-3 times a day.
Precautions
It is contraindicated for patients with spastic diseases, cerebral hemorrhage, severe arrhythmia, severe electrolyte imbalance, allergy to this product, intracranial tumors or other diseases that may cause increased intracranial pressure.
Drug Interactions
When used in combination with itraconazole, itraconazole can inhibit the metabolism of vincamine mediated by cytochrome P450. The combination can increase the neurotoxicity and paralytic ileus of vincamine, so it is necessary to monitor and adjust the dose of vincamine accordingly. When used in combination with quinupristin/dalfopristin, quinupristin/dalfopristin can inhibit the metabolism of vincamine mediated by cytochrome P450 and increase the toxicity of vincamine. When used in combination with vincamine, the dose of vincamine should be reduced or temporarily discontinued. Medical All Online/Online www.med126.com
