Isosorbide dinitrate sustained-release capsules
Function and Efficacy
The main pharmacological effect of isosorbide dinitrate (ISDN) is to relax vascular smooth muscle. ISDN is metabolized in the body to form isosorbide mononitrate (ISMN), which releases nitric oxide (NO). NO is the same as endothelial relaxing factor, activating guanylate cyclase, increasing cyclic guanosine monophosphate (cGMP) in smooth muscle cells, thereby relaxing vascular smooth muscle, dilating peripheral arteries and veins, and having a stronger dilating effect on veins. Venous dilation causes blood to remain in the periphery, reducing the amount of blood returning to the heart, and reducing left ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload). Arterial dilation reduces peripheral vascular resistance, systolic arterial pressure, and mean arterial pressure (afterload). Coronary artery dilation increases coronary perfusion. The overall effect is to reduce myocardial oxygen consumption, increase oxygen supply, and relieve angina pectoris. No carcinogenic and mutagenic phenomena were observed in animal experiments.

Appearance
This product is a capsule, and the contents are white or off-white powder.
Indication
Long-term treatment of coronary heart disease; prevention of angina pectoris; treatment of persistent angina pectoris after myocardial infarction; combined with digitalis and/or diuretics to treat chronic congestive heart failure; treatment of pulmonary hypertension.
Usage and Dosage
Tablets: 20~40 mg once, twice a day. Due to different individual reactions, the dose needs to be adjusted individually.
Adverse Reactions
In the early stage of medication, nitrate-induced vasodilatory headache may occur, as well as facial flushing, dizziness, orthostatic hypotension and reflex tachycardia. Occasionally, blood pressure may be significantly reduced, bradycardia and angina pectoris may be aggravated, and collapse and syncope may rarely occur.
Precautions
Acute circulatory failure (shock, circulatory collapse); severe hypotension (systolic blood pressure 90 mmHg); acute myocardial infarction with low filling pressures (unless under continuous hemodynamic monitoring); hypertrophic obstructive cardiomyopathy; constrictive pericarditis or cardiac tamponade; severe anemia; glaucoma; increased intracranial pressure; primary pulmonary hypertension; allergy to nitro compounds
Special Population Medication
Precautions for children: The safety and effectiveness of this product in children have not been established. Precautions for pregnancy and lactation: Women in the first 3 months of pregnancy are prohibited from using this product, and lactating women should use this product with caution. Precautions for the elderly: Not yet clear.
Drug Interactions
The combination of this drug with other vasodilators, calcium antagonists, beta-receptor blockers, antihypertensive drugs, tricyclic antidepressants and alcohol can enhance the blood pressure lowering effect of this drug. It can also enhance the pressor effect of dihydroergot alkaloids. The simultaneous use of steroid anti-inflammatory drugs can reduce the efficacy of this drug.
