Isosorbide mononitrate sustained-release capsules (Ⅰ)
Function and Efficacy
Isosorbide mononitrate (ISMN) is the main biologically active metabolite of isosorbide dinitrate. Like other organic nitrates, its main pharmacological action is to relax vascular smooth muscle. ISMN releases nitric oxide (NO), which is the same as endothelial relaxing factor, activating guanylate cyclase, increasing cyclic guanosine monophosphate (cGMP) in smooth muscle cells, thereby relaxing vascular smooth muscle, dilating peripheral arteries and veins, and having a stronger dilating effect on veins. Venous dilation causes blood to remain in the periphery, reducing the amount of blood returning to the heart, and reducing left ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload). Arterial dilation reduces peripheral vascular resistance, systolic arterial pressure, and mean arterial pressure (afterload). Coronary artery dilation increases coronary perfusion. The overall effect is to reduce myocardial oxygen consumption, increase oxygen supply, and relieve angina pectoris. No carcinogenic and mutagenic phenomena were observed in animal experiments.

Appearance
This product is a capsule, and the contents are white powder.
Indication
Long-term treatment of coronary heart disease; prevention of angina pectoris; treatment of persistent angina pectoris after myocardial infarction; combined with digitalis and/or diuretics to treat chronic congestive heart failure.
Usage and Dosage
Sustained-release capsule 50 mg, taken once daily after breakfast.
Adverse Reactions
Nitrate-induced vasodilatory headache may occur in the early stage of medication, but the symptoms usually disappear after taking it for several days. Facial flushing, dizziness, orthostatic hypotension and reflex tachycardia may also occur. Occasionally, blood pressure is significantly reduced, bradycardia, angina pectoris is aggravated and syncope may occur.
Precautions
- Patients who are allergic to nitro compounds are prohibited from taking this product. - Patients with acute circulatory failure (shock, circulatory collapse) are prohibited from taking this product. - Patients with cardiogenic shock (intra-aortic counterpulsation or positive inotropy cannot achieve sufficient left ventricular diastolic end pressure) are prohibited from taking this product. - Patients with severe hypotension (systolic blood pressure resists 90mmHg) are prohibited from taking this product. - It is prohibited to take sildenafil (trade name: Viagra) at the same time, because sildenafil will enhance the hypotensive effect of this product.
Special Population Medication
Precautions for children: All studies on this type of drug were conducted in adults. There is no data comparing the use of drugs in children and adults, so it is not recommended for use in children. Precautions for pregnancy and lactation: No toxic effects on embryos were observed in animal experiments, and it is not clear whether ISMN is excreted through breast milk. However, due to the lack of experience in the use of drugs in pregnant and lactating women, it should be used with caution. Precautions for the elderly: Elderly patients may be more sensitive to this type of drug and are more likely to experience reactions such as dizziness.
Drug Interactions
The concomitant use of vasodilators, antihypertensives, beta-blockers, calcium antagonists, tranquilizers, tricyclic antidepressants or alcohol can increase the hypertensive effect of this product. The concomitant use of nitrous oxide donors, such as isosorbide mononitrate (the active ingredient in this product) and sildenafil, can significantly enhance the hypotensive effect of this product. If this product is used in combination with dihydroergotamine, the blood pressure-raising effect of dihydroergotamine can be increased. Note: This product may also interact with previously used and upcoming drugs, so before using this product, you should inform your doctor or pharmacist of the drugs you have recently taken or are currently taking.
