Isosorbide Mononitrate Sustained Release Capsules
Function and Efficacy
Isosorbide mononitrate (ISMN) is the main biologically active metabolite of isosorbide dinitrate. Like other organic nitrates, its main pharmacological action is to relax vascular smooth muscle. ISMN releases nitric oxide (NO). NO is the same as endothelial relaxing factor, activating guanylate cyclase, increasing cyclic guanosine monophosphate (cGMP) in smooth muscle cells, thereby relaxing vascular smooth muscle, dilating peripheral arteries and veins, and having a stronger dilating effect on veins. Venous dilation causes blood to remain in the periphery, reducing the amount of blood returning to the heart, and reducing left ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload). Arterial dilation reduces peripheral vascular resistance, systolic arterial pressure, and mean arterial pressure (afterload). Coronary artery dilation increases coronary perfusion. The overall effect is to reduce myocardial oxygen consumption, increase oxygen supply, and relieve angina pectoris. Carcinogenicity and mutagenicity were not observed in animal experiments.

Appearance
This product is a hard capsule containing white micro-pellets.
Indication
Used to prevent angina pectoris; treat persistent angina pectoris after myocardial infarction and coronary heart disease; used in combination with digitalis or diuretics to treat chronic congestive heart failure.
Usage and Dosage
Oral administration: 10-20 mg at a time, twice a day, orally.
Adverse Reactions
1. Nitrate-induced vasodilatory headache may occur in the early stage of medication, but the symptoms usually disappear after taking it for several days. 2. Facial flushing, dizziness, orthostatic hypotension and reflex tachycardia may also occur. 3. Occasionally, blood pressure may drop significantly, bradycardia, angina pectoris may worsen and syncope may occur.
Precautions
1. Contraindicated in patients with acute circulatory failure (shock, circulatory collapse). 2. Contraindicated in patients with severe hypotension (systolic blood pressure 90 mmHg). 3. Contraindicated in patients with acute myocardial infarction and low filling pressure (unless under continuous hemodynamic monitoring). 4. Contraindicated in patients with hypertrophic obstructive cardiomyopathy, constrictive pericarditis or cardiac tamponade. 5. Contraindicated in patients with severe anemia, glaucoma, and increased intracranial pressure. 6. Contraindicated in patients allergic to nitro compounds.
Special Population Medication
Precautions for children: All studies on this type of drug were conducted in adults. There is no data comparing the use of drugs in children and adults, so it is not recommended for use in children. Precautions for pregnancy and lactation: No toxic effects on embryos were observed in animal experiments, and it is not clear whether ISMN is excreted through breast milk. However, due to the lack of experience in the use of drugs in pregnant and lactating women, it should be used with caution. Precautions for the elderly: Elderly patients may be more sensitive to this type of drug and are more likely to experience reactions such as dizziness.
Drug Interactions
Combination with other vasodilators, calcium antagonists, beta-receptor blockers, antihypertensives, tricyclic antidepressants and alcohol can enhance the blood pressure lowering effect of this class of drugs.
