Olanzapine
Indication
Olanzapine is suitable for the treatment of acute and maintenance phases of schizophrenia and other psychoses with severe positive or negative symptoms, and can also relieve secondary affective symptoms of schizophrenia and related diseases.
Usage and Dosage
1. Adults: (1) Schizophrenia: The recommended starting dose of olanzapine is 10 mg/day. Once a day. (2) Manic episode: The starting dose is 15 mg per day when used alone and 10 mg per day when used in combination therapy. (3) Prevention of relapse of bipolar disorder: The recommended starting dose is 10 mg/day. For patients using olanzapine to treat manic episodes, the continued treatment dose for prevention of relapse is the same as above. For new manic, mixed or depressive episodes, olanzapine treatment should be continued (with appropriate dose adjustment when necessary), and adjuvant medication should be combined to treat affective symptoms according to clinical conditions. During the prevention and treatment of schizophrenia, manic episodes and bipolar disorder, the daily dose can be adjusted accordingly within the range of 5-20 mg/day according to individual clinical conditions. It is recommended that drugs exceeding the recommended starting dose should be used only after appropriate clinical reassessment, and the interval between dosing should be no less than 24 hours. Olanzapine administration does not need to consider food factors, and food does not affect absorption. When discontinuing olanzapine, the dose should be gradually reduced. 2. Special populations: (1) Patients with impaired renal and/or liver function: A lower starting dose (5 mg/day) should be considered for such patients. The initial dose for patients with moderate hepatic impairment (cirrhosis, Child-Pugh grade A or B) should be 5 mg, and the dose should be increased with caution. (2) Smokers: Compared with smokers, the initial dose and dose range of non-smoking patients generally do not need to be adjusted. Smoking accelerates the metabolism of olanzapine, and clinical evaluation is recommended, and consideration of increasing the dose of olanzapine when necessary. When more than one factor that slows metabolism (female, elderly, non-smoking) is present, consideration should be given to lowering the starting dose. When the dose needs to be increased, it should also be conservative.
