Function and Efficacy

The main pharmacological action of isosorbide dinitrate (ISDN) is to relax vascular smooth muscle. ISDN is metabolized in the body to produce isosorbide mononitrate (ISMN), which releases nitric oxide (NO). NO is the same as endothelial relaxing factor, activating guanylate cyclase and increasing cyclic guanosine monophosphate (cGMP) in smooth muscle cells, thereby relaxing vascular smooth muscle, dilating peripheral arteries and veins, and having a stronger dilating effect on veins. Venous dilation causes blood to accumulate in the periphery, reducing the amount of blood returning to the heart, and reducing left ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload). Arterial dilation reduces peripheral vascular resistance, systolic arterial pressure, and mean arterial pressure (afterload). Coronary artery dilation increases coronary perfusion. The overall effect is to reduce myocardial oxygen consumption, increase oxygen supply, and relieve angina pectoris.

Appearance

The liquid of this product in the pressure-resistant container is a colorless, clear liquid. When the valve is pressed, the liquid will spray out in the form of mist particles.

Indication

It is used to prevent and treat various types of coronary heart disease and angina pectoris, especially for severe angina pectoris. When there are factors that induce angina pectoris such as mental stress, overwork, or the presence of premonitory symptoms, using this product in advance can prevent angina pectoris attacks.

Usage and Dosage

When using, first open the cap of the medicine bottle, the spray valve is at the top, the medicine bottle is vertical, press the spray valve several times until the spray is even, then use it. However, if it has not been used for a long time, you need to press the valve again until the spray is even before use. When using, point the spray nozzle at the mouth and press 4 times to achieve an effective dose of 2.5mg.

Adverse Reactions

In the early stage of medication, nitrate-induced vasodilatory headache may occur, as well as facial flushing, dizziness, orthostatic hypertension and reflex tachycardia. Occasionally, blood pressure may drop significantly. Bradycardia and angina pectoris may worsen, and collapse and syncope may occur rarely.

Precautions

Acute circulatory failure (shock, circulatory collapse); severe hypotension (systolic blood pressure < 90 mmHg); acute myocardial infarction with low filling pressure (unless under continuous hemodynamic monitoring); hypertrophic obstructive cardiomyopathy; constrictive pericarditis or cardiac tamponade; severe anemia; glaucoma; increased intracranial pressure; idiopathic pulmonary hypertension; allergy to nitro compounds.

Drug Interactions

Antihypertensive drugs, calcium antagonists, other venodilators, tricyclic antidepressants, neuroleptics or alcohol can enhance the antihypertensive effect of this drug. Concurrent use of dihydroergotamine can increase the blood concentration of dihydroergotamine.

Storage

Keep away from light and store in airtight container.
