Lidocaine Hydrochloride and Glucose Injection
Function and Efficacy
It is a class Ib antiarrhythmic. It mainly acts on Purkinje fibers and ventricular muscles, inhibiting Na influx and promoting K outflow; reducing the slope of phase 4 depolarization, thereby reducing automaticity; significantly shortening the action potential duration, relatively prolonging the effective refractory period and relative refractory period; reducing myocardial excitability; slowing down conduction velocity; and increasing the ventricular fibrillation threshold.
Appearance
Colorless or almost colorless clear liquid.
Indication
Used for ventricular arrhythmias caused by acute myocardial infarction, surgical operation, digitalis poisoning and cardiac catheterization, including ventricular premature beats, ventricular tachycardia and ventricular fibrillation.
Usage and Dosage
It is only used for intravenous drip maintenance after lidocaine hydrochloride injection loading intravenous injection treatment. The intravenous drip rate is 1-4 mg/min or 0.015-0.03 mg/kg/min. Elderly patients, patients with heart failure, cardiogenic shock, reduced liver blood flow, and liver and kidney damage should reduce the dose, and the intravenous drip rate should be controlled at 0.5-1 mg/min.
Adverse Reactions
The overall incidence is about 6%, most of which are related to the dose. 1 Nervous: dizziness, vertigo, nausea, vomiting, fatigue, paresthesia, muscle tremor, convulsions and respiratory depression. 2 Cardiovascular: The dose may produce severe sinus bradycardia, cardiac arrest, severe atrioventricular block and weakened myocardial contractility. 3 Allergic: manifested by rash and edema, and in severe cases, respiratory arrest.
Precautions
The following conditions should not be used: 1. Asperger's syndrome; 2. Severe heart block, including Ⅱ or Ⅲ° atrioventricular block. Bifascicular block; 3. Severe sinus node dysfunction; 4. Allergic to this product.
Storage
Keep in a sealed container. The validity period is tentatively set at one and a half years.
