Oxacillin Sodium Capsules
Function and Efficacy
This product is an acid-resistant and penicillinase-resistant penicillin. Oxacillin has good antibacterial activity against penicillinase-producing Staphylococci, but its antibacterial activity against various streptococci and non-penicillinase-producing Staphylococci is inferior to penicillin G. This product exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. [Pharmacokinetics] Oxacillin is acid-resistant, and 30% to 33% of the oral dose can be absorbed. The protein binding rate of oxacillin is 93%. Effective therapeutic concentrations can be reached in the liver, kidney, intestine, spleen, pleural effusion and joint cavity fluid, and the concentration is lower in ascites and sputum. Oxacillin is difficult to pass through the normal blood-cerebrospinal fluid barrier, but can pass through the placenta into the fetus, and a small amount is also secreted into breast milk. The elimination half-life (t1/2β) of this product in healthy adults is 0.4 to 0.7 hours; the elimination half-life (t1/2β) of newborns aged 8 to 15 days and 20 to 21 days is 1.6 days and 1.2 days respectively. About 49% of oxacillin is metabolized in the liver, and neither hemodialysis nor peritoneal dialysis can remove this product. [Overdose] The main manifestation of drug overdose is adverse reactions of the central nervous system. The drug should be discontinued in time and symptomatic and supportive treatment should be given. Hemodialysis cannot remove oxacillin.

Indication
Used for pneumonia and skin and soft tissue infections caused by penicillin-resistant Staphylococci.
Usage and Dosage
Oral administration on an empty stomach. Adults: For general infection, 0.5-1.0g (2-4 tablets) at a time; for severe patients, 1-1.5g (4-6 tablets) at a time, 3-4 times a day. Children: 70-100mg/kg per day, divided into 3-4 times. Newborns: For those weighing less than 2.5kg, 120mg per day; for those weighing more than 2.5kg, 160mg per day. Patients with mild to moderate renal impairment do not need to adjust the dose, but patients with severe renal impairment should avoid using large doses to prevent central nervous system toxicity.
Adverse Reactions
Mainly gastrointestinal reactions, such as nausea, vomiting, abdominal distension, diarrhea, loss of appetite, etc. Allergic reactions such as drug rash and drug fever can also be seen. In some cases, serum aminotransferase increases, and the symptoms disappear after stopping the drug. High-dose use may cause nervous system reactions, such as convulsions, cramps, confusion, headaches, etc. Occasionally, neutropenia or agranulocytosis may occur, which may cause bleeding tendency in people with special constitutions. Acute interstitial nephritis with renal failure has also been reported. A few cases may develop secondary infection with Candida albicans.
Precautions
It is contraindicated in patients with a history of allergy to penicillins or a positive penicillin skin test
