Function and Efficacy

This product is the dibenzylethylenediamine salt of penicillin, and its antibacterial active ingredient is penicillin. Penicillin has good antibacterial effects on hemolytic streptococci and other streptococci, Streptococcus pneumoniae and Staphylococci that do not produce penicillinase. It has moderate antibacterial effects on enterococci. Neisseria gonorrhoeae, Neisseria meningitidis, Corynebacterium diphtheriae, Bacillus anthracis, Actinomyces bovis, Streptobacillus moniliformis, Listeria monocytogenes, Leptospira and Treponema pallidum are sensitive to this product. This product also has certain antibacterial activity against Haemophilus influenzae and Bordetella pertussis. This product has good antibacterial effects on anaerobic bacteria such as Clostridium, Streptococcus and melanin-producing Bacteroides, but poor antibacterial effects on Bacteroides fragilis. Penicillin exerts its bactericidal effect by inhibiting bacterial cell wall synthesis.

Ingredients

This product is a sterile powder made by mixing penicillin dibenzylethylenediamine salt [(C16H18N2O4S)2·C16H20N2] with an appropriate amount of buffer and suspending agent. Molecular formula: (C16H18N2O4S)2·C16H20N2 Molecular weight: 909.14

Appearance

This product is white crystalline powder.

Indication

It is mainly used to prevent recurrence of rheumatic fever and can also be used to control the prevalence of streptococcal infections.

Usage and Dosage

Before use, add an appropriate amount of sterile water for injection to make a suspension. For intramuscular injection, adults should take 600,000 to 1.2 million units at a time, once every 2 to 4 weeks; children should take 300,000 to 600,000 units at a time, once every 2 to 4 weeks.

Adverse Reactions

1. Allergic reaction: Allergic reaction caused by penicillin may occur during the use of this product, among which rash is the most common, leukopenia, interstitial nephritis, asthma attack and serum sickness reaction are rare, and severe cases such as anaphylactic shock are occasionally seen; once anaphylactic shock occurs, it must be rescued on the spot, and the airway must be kept open, oxygen inhalation, and adrenaline, glucocorticoids and other treatment measures must be taken. 2. Superinfection: Superinfection with penicillin-resistant Staphylococcus aureus, Gram-negative bacilli or Candida albicans may occur.

Precautions

It is contraindicated in patients with a history of allergy to penicillins or a positive penicillin skin test.

Special Population Medication

Precautions for children: See Dosage and Administration. Precautions for pregnancy and lactation: Animal reproduction tests have not found that penicillin causes fetal damage, but strict controlled tests have not been conducted in pregnant women to rule out the adverse effects of such drugs on the fetus, so pregnant women should only use this product when it is really necessary. A small amount of this product is secreted from breast milk, so breastfeeding should be suspended when breastfeeding women take the drug. Precautions for the elderly: This experiment has not been conducted and there are no reliable references.

Drug Interactions

Probenecid, aspirin, indomethacin, phenylbutazone and sulfonamides reduce the renal tubular secretion of penicillin and prolong the serum half-life of this product. Penicillin can enhance the anticoagulant effect of warfarin.
