Compound Glycyrrhizin Capsules

Function and Efficacy

Pharmacological effects: 1. Anti-inflammatory effects (1) Anti-allergic effects: Glycyrrhizic acid glycoside has anti-allergic effects such as inhibiting local allergic reactions (Arthus Phenomenon) in rabbits and inhibiting Schwartzman phenomenon (Shwartzman Phenomenon). For corticosteroids, it enhances the hormone's inhibitory effect on stress response and antagonizes the hormone's anti-granulation and thymic atrophy effects. It has no effect on the exudation of hormones. (2) Inhibitory effect on arachidonic acid metabolic enzymes Glycyrrhizic acid glycoside can directly bind to phospholipase A2, the initiator enzyme of the arachidonic acid metabolic pathway, and to lipoxygenase, which acts on arachidonic acid to produce inflammatory mediators, selectively inhibiting the phosphorylation of these enzymes and inhibiting their activation. 2. Immunomodulatory effect: Glycyrrhizic acid glycoside has the following immunomodulatory effects in in vitro tests: (1) Regulation of T cell activation; (2) Induction of gamma interferon; (3) Activation of NK cells; (4) Promotion of extrathymic T lymphocyte differentiation. 3. Inhibition of experimental hepatocyte damage In the rat hepatocyte cell line cultured in invitro, glycyrrhizic acid glycoside has the effect of inhibiting hepatocyte damage caused by carbon tetrachloride. 4. Inhibition of viral proliferation and inactivation of viruses In the mouse MHV (mouse hepatitis virus) infection experiment, glycyrrhizic acid glycoside can prolong the survival of mice; in the rabbit vaccinia virus (Vacciniavirus) pox prevention experiment, it has the effect of preventing pox; in the in vitro experimental system, the inhibition of the proliferation of herpes virus and the inactivation of viruses were also observed. It has been reported that glycine and methionine can inhibit the decrease in urine volume and sodium excretion caused by oral administration of glycyrrhizic acid glycoside to rats.

Appearance

This product is a capsule, and the contents are light yellow granules.

Indication

Treat chronic liver disease and improve abnormal liver function. Can be used to treat eczema, dermatitis and alopecia areata.

Usage and Dosage

Adults usually take 2-3 capsules at a time, and children take 1 capsule at a time, 3 times a day, orally after meals. The dosage can be increased or decreased according to age and symptoms.

Adverse Reactions

After large amounts of glycyrrhizic acid are administered for a long time, pseudoaldosteronism such as hypokalemia, increased blood pressure, blood sodium and fluid retention, edema, and weight gain can be seen. As for its pathogenesis, Kumagai Akira believes that it is because glycyrrhizic acid and its metabolite glycyrrhetinic acid inhibit the activity of the Δ4-3Ketocteroid metabolic enzyme Δ4-reductase (Δ4-reductase) in the liver; Stewort believes that it is because glycyrrhetinic acid inhibits the activity of the metabolic enzyme 11β-HCD (11-β-hydroxysteroid dehydrogenase) that converts cortisol into cortisone in the kidney, causing excess hydrocortisone to replace aldosterone and bind to the kidney type I receptor, thereby exerting a hydrocorticoid-like effect. When healthy people are given glycyrrhizic acid for a long time, different degrees of hydrocorticoid-like effects can also be seen. In addition, there are also reports that glycyrrhizic acid inhibits the activity of 11β-HCD, leading to abnormal steroid metabolism [9]. However, the occurrence of pseudoaldosteronism is limited to patients who have been given large amounts of licorice or glycyrrhizic acid for a long time, and it is also related to individual differences in these patients [10]. In fact, in the double-blind treatment of chronic hepatitis patients with SNMC, 80 mg of glycyrrhizic acid was intravenously injected daily for 4 weeks, and no pseudoaldosteronism was observed. This is related to the route of administration and dosage, as well as the compound preparation. The potential pseudoaldosterone effect of glycyrrhizic acid can be weakened by the glycine and L-cysteine contained in the compound preparation. Adverse reactions are rarely seen with a large dose of 100 ml of SNMC per day. Hino reported that 1 case of hypertension (1.5%) and 5 cases of hypokalemia (7.5%) were found in 65 patients, but they were all mild cases, and the blood potassium values were all above 3.0 mEg/L. No serious adverse reactions requiring drug discontinuation were observed. Therefore, when using compound glycyrrhizin injection and compound glycyrrhizin tablets, it is necessary to regularly check electrolytes (especially serum potassium values) and measure blood pressure. When serum potassium decreases, drug discontinuation may be considered. In cases of obvious hypokalemia, potassium treatment is often given, but it is found that urinary potassium excretion also increases. When correction of hypokalemia is not obvious, anti-aldosterone preparations (potassium-sparing diuretics) may also be given to increase blood potassium and correct the increase in blood pressure.

Precautions

(The following patients should not be given this medicine) 1. Patients with aldosteronism, myopathy, hypokalemia (it may aggravate hypokalemia and hypertension). 2. Patients with advanced liver cirrhosis who tend to have elevated blood ammonia (the metabolites of methionine contained in this preparation can inhibit urea synthesis, thus reducing the ability to handle ammonia).

Special Population Medication

Precautions for children: Children take 1 tablet at a time, 3 times a day, orally after meals. Precautions for pregnancy and lactation: Pregnant and lactating women should use the drug with caution after weighing the benefits of treatment outweigh the risks. Precautions for the elderly: Based on clinical application experience, the elderly are prone to adverse reactions of hypokalemia, so they need to be administered with caution under close observation.

Drug Interactions

1. Precautions when using combined medications: When used in combination with loop diuretics, uric acid, furosemide, thiazide and antihypertensive diuretics, trichloromethiazide, chlorthalidone, etc., hypokalemia (fatigue, low muscle strength) may occur, and full attention should be paid to observing serum potassium values. 2. Mechanism and consequences: Diuretics can enhance the potassium excretion effect of glycyrrhizic acid contained in the preparation, further lowering serum potassium.
