Compound Pueraria Hydrochlorothiazide Tablets
Function and Efficacy
This compound preparation has a hypotensive effect. Hydrochlorothiazide has a diuretic effect, increasing the excretion of urinary sodium, potassium, chloride, phosphorus and magnesium ions, while reducing the excretion of urinary calcium. The mechanism of action of this type of drug is mainly to inhibit the reabsorption of sodium chloride in the anterior segment of the distal tubule and the proximal tubule (with a milder effect), thereby increasing the Na+-K+ exchange in the distal tubule and the collecting duct, and increasing the secretion of K+. Its mechanism of action is not yet fully understood. This type of drug can inhibit the activity of carbonic anhydrase to varying degrees, so it can explain its effect on the proximal tubule. This type of drug can also inhibit the activity of phosphodiesterase, reduce the uptake of fatty acids by the renal tubule and mitochondrial oxygen consumption, thereby inhibiting the active reabsorption of Na+ and Cl- by the renal tubule. In addition to its diuretic and natriuretic effects, hydrochlorothiazide may also have an extrarenal mechanism that participates in lowering blood pressure, which may be to increase the gastrointestinal excretion of Na[sup]+[/sup]. Through diuretic and natriuretic effects, clonidine hydrochloride can excite the α2 adrenergic receptors on the postsynaptic membrane of the medulla oblongata, excite inhibitory neurons, reduce the outflow of central sympathetic nerve impulses, and thus inhibit peripheral sympathetic nerve activity. Clonidine also excites the α2 receptors on the presynaptic membrane of peripheral sympathetic nerves, enhancing their negative feedback, reducing the release of norepinephrine from peripheral nerves, reducing peripheral vascular and renal vascular resistance, slowing heart rate, and lowering blood pressure. Renal blood flow and glomerular filtration rate remain basically unchanged. Orthostatic symptoms are mild or rare, and postural hypotension rarely occurs.
