Isosorbide dinitrate cream

Function and Efficacy

The main pharmacological action of isosorbide dinitrate (ISDN) is to relax vascular smooth muscle. ISDN is metabolized in the body to produce isosorbide mononitrate (ISMN), which releases nitric oxide (NO). NO is the same as endothelial relaxing factor, activating guanylate cyclase and increasing cyclic guanosine monophosphate (cGMP) in smooth muscle cells, thereby relaxing vascular smooth muscle, dilating peripheral arteries and veins, and having a stronger dilating effect on veins. Venous dilation causes blood to accumulate in the periphery, reducing the amount of blood returning to the heart, and reducing left ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload). Arterial dilation reduces peripheral vascular resistance, systolic arterial pressure, and mean arterial pressure (afterload). Coronary artery dilation increases coronary perfusion. The overall effect is to reduce myocardial oxygen consumption, increase oxygen supply, and relieve angina pectoris.

Ingredients

Main ingredients and chemical name: 1,4:3,6-dianhydro-D-sorbitol dinitrate.

Appearance

This product is a white or milky white cream with an emulsion base.

Indication

1. Treat and prevent angina pectoris. 2. Can also be used to treat heart failure when digitalis or diuretics are not effective enough.

Usage and Dosage

It is advisable to start with a small dose and gradually increase the dose. Squeeze the cream to the required length according to the scale, and evenly apply it on the paper with scales printed on it. Each grid is equivalent to 0.2g of isosorbide dinitrate. Apply the entire area of the paper, that is, an area of 5cm×5cm, and stick it on the left chest area (fixed with adhesive tape) once a day (once every 8 hours if necessary) and before going to bed.

Adverse Reactions

1 Headache, which is easy to occur when first used. 2 Low blood pressure, accompanied by dizziness, nausea, vomiting, sweating and even collapse. 3 Facial flushing and body heat. 4 Occasionally, skin rash or even exfoliative dermatitis occurs.

Precautions

Acute circulatory failure (shock, circulatory collapse); severe hypotension (systolic blood pressure < 90 mmHg); acute myocardial infarction with low filling pressures (unless under continuous hemodynamic monitoring); hypertrophic obstructive cardiomyopathy; constrictive pericarditis or cardiac tamponade; severe anemia; glaucoma; increased intracranial pressure; primary pulmonary hypertension; allergy to nitro compounds

Special Population Medication

Precautions for children: The safe and effective treatment range for children has not yet been found. Precautions for pregnancy and lactation: Pregnant women can use this drug when needed. It is not clear whether it is secreted in breast milk. Precautions for the elderly: There is no abnormality in the use of this drug by the elderly.

Drug Interactions

1. Isosorbide dinitrate can enhance the effects of other vasodilators. When used in combination with other antihypertensive drugs, the orthostatic hypotensive effect of isosorbide dinitrate can be more prominent. 2. Indomethacin can inhibit the vasodilatory effect of isosorbide dinitrate.

Storage

Keep away from light and store in airtight container.
