NSI-189 is an experimental, potential antidepressant that was developed by Neuralstem, Inc. for the treatment for major depressive disorder, as well as for cognitive impairment and neurodegeneration.

^{At present, NSI189 is still being tested for other conditions such as improved brain health, diabetic neuropathy, stroke, neurodegenerative disease, post-traumatic stress disorder, and Angelman syndrome. It is used to treat traumatic brain injury.‍}

On Human Studies

• Improved depression.

• In a placebo-controlled study, taking 40 mg to 80 mg per day of NSI 189 for 28 days showed significant improvements to depression symptoms in several people.
• Improved cognition.

• Self-reported improvements in cognition were reported by those under the phase I trial. This includes not only memory but also learning ability and focus.

On Animal Studies

The supplement revealed more of its effects that are favorable to humans.

• Ischemic stroke. A study on rats resulted in improved brain and motor deficits after an induced stroke. NSI-189 reverses cell death that is caused by oxygen deprivation. The effect lasted 24 weeks.
• Diabetic neuropathy. A person with diabetes may have neuropathy, which is a form of nerve damage. To help regenerate the nerve, NSI-189 aids in the growth of neurons and stops further damage to the brain and limbs.
• Angelman syndrome. The nervous system is the most affected part of the body because of Angelman syndrome. It is a genetic disorder that causes cognitive and motor skill impairments. NSI 189 activates TrkB and Akt pathways and reverses such impairments.

Most of the clinical research on NSI 189 is on animals, and there are only very few on humans.

Nonetheless, patients' side effects are reported in small or higher doses regardless of taking it.

People who use the drug at 40 mg may still experience dry mouth, restlessness, sleepiness, headaches, dizziness, rash, hyperthermia, and palpitations.

There are no serious long-term side effects reported on animals when administered in high doses.
