    Description

Tofacitinib, also known as tasocitinib, CP-690550, is a Janus kinase (JAK) inhibitor. Tofacitinib modulates the signaling pathway at the point of JAKs, preventing the phosphorylation and activation of STATs. JAK enzymes transmit cytokine signaling through pairing of JAKs (e.g., JAK1/JAK3, JAK1/JAK2, JAK1/TyK2, JAK2/JAK2). Tofacitinib inhibited the in vitro activities of JAK1/JAK2, JAK1/JAK3, and JAK2/JAK2 combinations with IC50 of 406, 56, and 1377 nM, respectively.

    Tofacitinib Citrate More Info

Introduction:  Tofacitinib citrate is an oral medication that belongs to a class of drugs known as Janus kinase (JAK) inhibitors. It is primarily used to treat autoimmune diseases, particularly rheumatoid arthritis (RA), psoriatic arthritis (PsA), and ulcerative colitis (UC). Tofacitinib works by modulating the immune system to reduce inflammation and symptom severity.

Mechanism of Action:  Tofacitinib selectively inhibits the activity of JAK enzymes (specifically JAK1 and JAK3). These enzymes play a critical role in the signaling pathways of several cytokines and growth factor receptors involved in the immune response. By inhibiting JAK, tofacitinib reduces the activity of these cytokines, leading to decreased inflammation and a subsequent improvement in symptoms.

Indications:  Tofacitinib is used for the treatment of adults with moderate to severe active rheumatoid arthritis who have had an inadequate response to one or more disease-modifying antirheumatic drugs. It is also approved for the treatment of adults with active psoriatic arthritis and indicated for adults with ulcerative colitis who have had an inadequate response to traditional therapies, including corticosteroids and TNF inhibitors.

Dosage:  The typical starting dose for rheumatoid arthritis is 5 mg taken orally twice daily, with adjustments based on clinical response and tolerability. For psoriatic arthritis, the recommended dose is similar. In ulcerative colitis, a starting dose of 10 mg twice daily is usually given for the first 8 weeks, followed by a maintenance dose of 5 mg or 10 mg twice daily.

Side Effects:  Common side effects of tofacitinib include upper respiratory infections, headache, diarrhea, nausea, and elevated liver enzymes. Serious side effects can include increased risk of serious infections, blood clots, changes in blood lipid levels, and liver function abnormalities.

Precautions:  Patients should be screened for infections before starting therapy, with particular attention to latent tuberculosis. Routine monitoring of liver function tests is recommended, and regular monitoring of complete blood counts is advised, as tofacitinib may cause decreased blood cell counts.

Drug Interactions:  Tofacitinib may interact with other medications, including other immunosuppressants, strong CYP3A4 inhibitors or inducers, and live vaccines, which should be avoided during treatment.

Contraindications:  Tofacitinib should not be used in patients with active infections or serious infections, severe liver impairment, or known hypersensitivity to tofacitinib or any of its components.

Discontinuation:  Patients are advised not to discontinue tofacitinib without consulting their healthcare provider, especially if they have underlying autoimmune conditions that may require continuous treatment.

Conclusion:  Tofacitinib citrate is an important therapeutic option for chronic inflammatory conditions such as rheumatoid arthritis, psoriatic arthritis, and ulcerative colitis. Like all medications, it should be used under the supervision of a healthcare professional, considering potential side effects and the need for ongoing monitoring. Regular follow-up appointments are essential to assess treatment efficacy and safety.
