Linagliptin, also known as BI-1356, is a DPP-4 inhibitor developed by Boehringer Ingelheim for treatment of type II diabetes. Linagliptin (once-daily) was approved by the US FDA on 2 May 2011 for treatment of type II diabetes.

Mechanism of Action

Linagliptin inhibits the DPP-4 enzyme, which degrades incretin hormones like glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP). By blocking DPP-4, linagliptin increases incretin hormone levels, stimulating insulin release in a glucose-dependent manner and reducing glucagon levels. Linagliptin binds tightly, but not irreversibly, to the DPP-4 enzyme, providing a 24-hour inhibition profile.

Applications

Linagliptin is used to treat type 2 diabetes in adults. It helps lower blood sugar levels by:

Low blood sugar (hypoglycemia): Symptoms include headache, crankiness, anxiety, hunger, dizziness, confusion, blurry vision, slurred speech, sweating, shakiness, fast heartbeat, and seizures.
Pancreatitis (inflammation of the pancreas): Symptoms include vomiting or severe pain in the upper stomach that travels to the back.
Heart failure: Symptoms include shortness of breath (especially while lying down), swelling in the feet/ankles/legs, unusually fast weight gain, and unusual tiredness.
Severe joint pain: New or increased joint pain.
Severe skin reactions: Rash or blisters on the skin.

Before taking linagliptin, tell your doctor if you have ever had:
Severe allergic reaction to linagliptin
Heart problems
Kidney disease
Pancreatitis
High triglycerides
Gallstones
Linagliptin may cause severe, life-threatening pancreatitis. Stop taking linagliptin and call your doctor if you have pain in your upper stomach that may spread to your back, with or without vomiting.
Tell your doctor if you are pregnant or breastfeeding. Control of diabetes is very important during pregnancy.

Pharmacokinetics

Linagliptin has a xanthine-based structure, which may account for some of its pharmacological differences compared to other DPP-4 inhibitors. It is a potent and selective inhibitor of DPP-4, with high selectivity for DPP-4 compared with the enzymes DPP-8 and DPP-9. Linagliptin is a weak-to-moderate inhibitor of cytochrome P450 enzymes.
