Fasudil hydrochloride is a medication primarily known as a ROCK (Rho-associated protein kinase) inhibitor. It's used to treat a variety of conditions, including cerebral vasospasm after subarachnoid hemorrhage. It can also be used as an antihypertensive agent and a vasodilator. 

Applications

Cardiovascular and cerebrovascular drugs are used to improve symptoms of ischemic cerebrovascular diseases caused by cerebral vasospasm after subarachnoid hemorrhage.

Mechanism of action

In vitro experiments have shown that this product can relax isolated cerebral blood vessels, inhibit the contraction of isolated blood vessels caused by calcium ion influx, suppress the contraction effect of various different mechanisms of cerebral vasoconstrictors, inhibit intracellular calcium ions, and induce vascular contraction, but it will not reduce intracellular calcium ion concentration. Clinically used to improve and prevent cerebral vasospasm and subsequent ischemic symptoms after subarachnoid hemorrhage surgery. Administer 30mg intravenously once, 2-3 times a day. This agent should be used in the early postoperative period of subarachnoid hemorrhage, generally for 2 weeks. Dilute with 50-100mL of NS or GS before intravenous infusion, with each infusion lasting for 30 minutes. For patients with renal insufficiency, the dosage should be appropriately reduced, such as 10mg once. Adverse reactions after use may include intracranial hemorrhage, gastrointestinal hemorrhage, pulmonary hemorrhage, nosebleeds, subcutaneous hemorrhage, and consciousness disorders. There is also abnormal liver function. Occasional low blood pressure, anemia, leukopenia, renal dysfunction, polyuria, rash, fever, etc. Patients with intracranial hemorrhage or potential intracranial hemorrhage, as well as those with hypotension, are prohibited from using it. Preoperative patients with diabetes, cerebral arterial sclerosis, severe disturbance of consciousness, subarachnoid hemorrhage with cerebrovascular disorder, liver and kidney dysfunction, patients over 70 years old, pregnant women and children should use caution. Breastfeeding women should stop breastfeeding when taking medication. This medicine can only be administered intravenously and cannot be administered through other routes.
