Capecitabine  is an oral chemotherapy medication used to treat several types of cancer. As a prodrug, it is converted into the active cancer-killing agent 5-fluorouracil (5-FU) inside the body, particularly within tumor tissue. 

Mechanism of action

Capecitabine is activated in a three-step process that occurs primarily in tumor cells, where high levels of the enzyme thymidine facilitate the final conversion. 

Capecitabine is first converted to 5'-deoxy-5-fluorocytidine (5'-DFCR).

This intermediate is then metabolized to 5'-deoxy-5-fluorouridine (5'-DFUR).

Finally, 5'-DFUR is converted to the active cytotoxic agent 5-fluorouracil (5-FU).

Uses

Capecitabine is prescribed to treat various cancers, often alone or in combination with other drugs.

Colorectal cancer: Used to treat metastatic colorectal cancer and as an adjuvant therapy following surgery for stage III colon cancer.

Breast cancer: Treats metastatic breast cancer, especially in cases where the disease is resistant to other chemotherapies like anthracyclines and taxanes.

Gastric, esophageal, and gastroesophageal junction cancer: Used in combination with other therapies to treat advanced or metastatic forms of these cancers.

Pancreatic cancer: Prescribed as adjuvant therapy after surgery to help prevent recurrence.

Side Effects

The most frequently reported side effects of capecitabine include: 

Hand-foot syndrome (redness, swelling, pain, or blistering of the hands and feet)

Diarrhea

Nausea and vomiting

Fatigue

Abdominal pain

Low blood cell counts (leading to an increased risk of infection, bleeding, and anemia)

Serious warnings and side effects:

Severe diarrhea: A significant increase in bowel movements, which can lead to severe dehydration.

Heart problems: Chest pain, palpitations, or shortness of breath. This risk may be higher in patients with a history of heart disease.

Infection: Fever, chills, or sore throat.

Blood clotting problems: Swelling, warmth, or pain in a leg or arm.

Severe skin reactions: Blistering or peeling skin, which could indicate a serious condition like Stevens-Johnson syndrome.

Liver injury: Yellowing of the skin or eyes , dark urine, or abdominal pain.
