Description	Scopolamine is an alkaloid from SOLANACEAE, especially DATURA and SCOPOLIA. Scopolamine and its quaternary derivatives act as antimuscarinics like ATROPINE, but may have more central nervous system effects. Among the many uses are as an anesthetic premedication, in URINARY INCONTINENCE, in MOTION SICKNESS, as an antispasmodic, and as a mydriatic and cycloplegic.

Scopolamine More Info

Scopolamine is a naturally occurring tropane alkaloid derived from plants of the Solanaceae family, such as Datura stramonium and Scopolia carniolica. It is structurally similar to atropine and functions as a non-selective competitive antagonist of muscarinic acetylcholine receptors (M1–M5), exerting both central and peripheral anticholinergic effects .

Mechanism of Action:

Scopolamine works by blocking cholinergic transmission in the central nervous system, particularly by inhibiting signals from the vestibular nuclei to higher CNS centers and from the reticular formation to the vomiting center, which helps prevent motion-induced nausea and vomiting . It also reduces gastrointestinal motility and secretions, and causes sedation, pupillary dilation, and increased heart rate .

Clinical Uses:

Prevention of motion sickness via transdermal patch (e.g., Transderm Scop)

Postoperative nausea and vomiting

Premedication before anesthesia to reduce saliva and respiratory secretions

Historically used for twilight sleep in obstetrics and to treat Parkinsonian tremors

Side Effects:

Common effects include drowsiness, dry mouth, blurred vision, and dizziness. At higher doses, it may cause hallucinations, delirium, or excitation .

Pharmacokinetics:

When applied via transdermal patch, scopolamine is absorbed slowly, reaching peak plasma levels in about 24 hours, with effects lasting up to 3 days. It crosses the blood-brain and placental barriers, and is metabolized extensively, with <10% excreted unchanged in urine.
