    Description

Nevirapine is a non-nucleoside reverse transcriptase inhibitor (NNRTI) used to treat HIV-1 infection and AIDS. As with other antiretroviral drugs, HIV rapidly develops resistance if nevirapine is used alone, so recommended therapy consists of combinations of three or more antiretrovirals. Nevirapine in triple combination therapy has been shown to suppress viral load effectively when used as initial antiretroviral therapy (i.e., in antiretroviral-naive patients).

    Nevirapine More Info

Nevirapine (NVP) is a non-nucleoside reverse transcriptase inhibitor (NNRTI), an oral antiviral medication primarily used for the treatment and prevention of human immunodeficiency virus type 1 (HIV-1) infection. It is widely applied in combination antiretroviral therapy (cART) globally, especially in resource-limited regions due to its cost-effectiveness and convenient administration.

    Core Medical Uses

Treatment of HIV-1 InfectionNevirapine is a core component of first-line cART regimens for both adults and pediatric patients (aged ≥15 days) with HIV-1 infection. It is used in combination with two nucleoside/nucleotide reverse transcriptase inhibitors (NRTIs) to suppress viral replication, reduce viral load, restore immune function, and delay the progression of acquired immunodeficiency syndrome (AIDS).

Prevention of Mother-to-Child Transmission (PMTCT) of HIVIt is a key drug for PMTCT programs. Pregnant women living with HIV can take nevirapine during labor and delivery, and newborns receive a single dose within 72 hours of birth. This short-course regimen significantly lowers the risk of HIV transmission from mother to child through breastfeeding or perinatal exposure.

    Mechanism of Action

Unlike NRTIs, nevirapine does not require intracellular phosphorylation to be active. It binds directly to the allosteric site of HIV-1 reverse transcriptase (RT), inducing a conformational change in the enzyme. This inhibits RT’s ability to convert viral RNA into DNA, thereby blocking HIV replication and preventing the virus from integrating into host cell DNA.

    Key Specifications & Administration

Dosage Forms: Film-coated tablets (200 mg per tablet), oral suspension (50 mg/5 mL), pediatric drops (10 mg/mL).

Administration: Oral, twice daily for adults (initial dose: 200 mg once daily for the first 14 days to reduce rash risk; maintenance dose: 200 mg twice daily). Pediatric dosage is based on body weight. It can be taken with or without food.

Storage: Store at 15–30°C (59–86°F). Protect from light and moisture. Keep out of reach of children.

Quality Standards: Complies with international pharmacopoeias such as USP, EP, and BP to ensure API purity and safety.

    Adverse Effects & Precautions

    Common Adverse Effects

Skin Reactions: Mild to moderate rash (maculopapular rash) is the most frequent side effect, usually occurring within the first 6 weeks of treatment. It typically resolves with continued use.

Hepatotoxicity: Elevated liver enzymes may occur, especially in the first 12 weeks.

Systemic Symptoms: Headache, fatigue, nausea, diarrhea, and fever are common and generally mild.

    Serious Precautions

Severe Skin Reactions: Rare but life-threatening reactions include Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN). Discontinue the drug immediately if a rash is accompanied by fever, blistering, or mucosal lesions.

Severe Hepatotoxicity: May lead to fulminant hepatitis or liver failure, particularly in patients with high baseline viral load or co-infection with hepatitis B/C. Monitor liver function regularly during the first 18 weeks of treatment.

Contraindications: Hypersensitivity to nevirapine or any of its excipients; severe liver dysfunction.

    Pharmaceutical Trade Notes

As an essential HIV treatment API, nevirapine has stable global demand, especially in low- and middle-income countries participating in HIV prevention programs. Suppliers need to comply with strict regulatory requirements (e.g., WHO Prequalification, FDA approval) to ensure product quality and access to international tenders.
