Calcium Polystyrene Sulfonate is an oral or enema-administered drug for lowering blood potassium levels. Its common dosage form is powder. This drug is mainly used for preventing and treating hyperkalemia caused by acute and chronic renal insufficiency and renal failure.

Pharmacological Effects
Calcium Polystyrene Sulfonate is not digested or absorbed when taken orally. In the intestine (mainly in the colon area), its calcium ions exchange with potassium ions in the intestinal tract. The resin itself does not undergo chemical changes and is excreted in its original form along with feces, thereby removing potassium ions from the intestinal tract and eliminating them from the body, achieving the purpose of reducing serum potassium concentration.
The data from in vitro experiments show that each 1 gram of calcium polystyrene sulfonate can displace approximately 53 to 71 milligrams (1.36 to 1.82 milliequivalents) of potassium ions.

Clinical Application
Hyperkalemia is a clinical emergency, especially common in patients with chronic kidney disease, and it may lead to severe arrhythmias or even cardiac arrest.
Calcium Polystyrene Sulfonate has important applications in this field. A clinical study involving 120 patients with hyperkalemia in chronic kidney disease showed that the treatment group using calcium polystyrene sulfonate had lower blood potassium levels at 1 week, 2 weeks, and 4 weeks after treatment compared to the control group. The total effective rate (95.00%) was higher, and the incidence of adverse reactions (8.33%) was not significantly different from that of the control group (6.67%), indicating that it can more quickly lower blood potassium levels, improve efficacy, and have good safety.
In addition to patients with renal dysfunction, this drug can also be used in situations where blood potassium levels rise due to major surgical procedures, trauma, burns, severe infections, etc.
Usage and Dosage
The usual adult dosage is 20 grams per day (usually divided into 1 to 3 doses), and the dosage for children is 5 to 10 grams per day. When taking the medicine, mix the powder with about 150 milliliters of water, stir well, and then take it immediately.
Precautions and Contraindications
When using this product, it is necessary to monitor serum potassium and serum calcium concentrations. When the serum potassium concentration drops to 4 to 5 milliequivalents per liter, the medication should be stopped.
To prevent drug interactions, it should be noted that it may enhance the effect of digoxin poisoning. When used in combination, caution is required.
In cases of severe hyperkalemia (serum potassium > 6.5 milliequivalents per liter) with changes in the electrocardiogram, other potassium-lowering measures should be combined.
Contraindications include patients with low blood potassium and high blood calcium.
Patients with hyperparathyroidism and multiple myeloma should use it with caution.
Patients with specific metabolic disorders such as intolerance to fructose should not take it together with sorbitol.
During medication, gastrointestinal reactions such as constipation, nausea, vomiting, and abdominal discomfort may occur, which are normal adverse reactions.
