Travoprost is a prostaglandin analog used primarily to reduce elevated intraocular pressure (IOP) in patients with open-angle glaucoma or ocular hypertension. It is also approved for pediatric patients aged two months and older.

Mechanism of Action

Travoprost is an isopropyl ester prodrug that, after ophthalmic administration, is absorbed through the cornea and hydrolyzed by esterases to its active form, travoprost free acid. The active metabolite acts as a full agonist at the prostaglandin F2-alpha (FP) receptor. By binding to this receptor, it increases the outflow of aqueous humor through both the trabecular meshwork and uveoscleral pathways, thereby lowering intraocular pressure. This mechanism helps prevent optic nerve damage and vision loss associated with high IOP in glaucoma and ocular hypertension.

Clinical Uses

Open-angle glaucoma: Lowers IOP to reduce risk of optic nerve damage and vision loss.
Ocular hypertension: Used to manage elevated IOP in patients without glaucoma.

Dosage and Administration

Formulation: Ophthalmic solution (0.004%) used as eye drops.
Typical dose: One drop in the affected eye(s) once daily in the evening.
Implant: Also available as an implant inserted into the eye by a healthcare professional.
Onset and duration: IOP reduction begins about 2 hours after dosing, with maximum effect at 12 hours; effect persists for up to 84 hours post-dose.
