PEG-MGF is a synthetic analog of "mechano-growth factor" (MGF) that has been modified via "PEGylation"—the attachment of polyethylene glycol (PEG) chains. MGF is a splice variant of insulin-like growth factor-1 (IGF-1), produced by skeletal muscle in response to mechanical stress or injury. While endogenous MGF degrades within minutes in plasma, PEGylation extends its half-life to 24–72 hours. This modification not only facilitates systemic administration but also enables its use in preclinical research concerning muscle regeneration, tissue repair, bone healing, and cardiac recovery.

MGF is generated from the IGF-1 gene through alternative splicing, with its production triggered by mechanical loading or tissue damage. Its primary function is to activate quiescent satellite cells—muscle stem cells used by the body to repair damaged muscle fibers—and to increase the population of myogenic progenitor cells before they are induced to differentiate by mature IGF-1. The E-peptide domain of MGF possesses biological activity independent of the IGF-1 receptor, positioning PEG-MGF not merely as an anabolic stimulant, but as a coordinator of the entire repair cascade.

The half-life of endogenous MGF in plasma is approximately 5 to 7 minutes. Through PEGylation (the attachment of polyethylene glycol chains), this half-life is extended to approximately 24–72 hours.
