Albumins, blood plasma Use and Manufacturing

Methods of Manufacturing

Method 1. Use human placental blood as raw material to process, immerse and extract, and salt out with human placental gamma globulin. Acidic precipitation, dialysis Take salting out to remove the ammonium sulfate mother liquor of precipitation (for the production of gamma globulin), adjust the pH to 4.3-4.4, leave it for 1 hour, filter on canvas to take the precipitate. Pack the precipitate in a cellophane dialysis bag, leave a certain volume, remove the air, tie the mouth, and dialyze with convection water until the ammonium sulfate content is less than 2g/L. Filtrate [HCl]→[pH4.3-4.4] precipitate [dialysis]→[to flowing water]→dialysis solution heat treatment, salting out, add the dialysate to sodium chloride (make the concentration reach 8.5 g/L), stir and dissolve, canvas Filter, measure the protein content of the filtrate, dilute with pyrogen-free water to make the protein concentration 12-20g/L. Add solid sodium caprylate to 2g/L, 1mol/L hydrochloric acid to adjust pH 5-5.2, stir to dissolve, 0-5℃ overnight. On the next day, heating in a water bath with pH 5-5.2 and 67°C, adding activated carbon (0.2%) and stirring for 15min, heating up to 69°C, holding for 1h, cooling to below 45°C, filtering, and taking the filtrate. Add sodium chloride or ammonium sulfate to 178g/L by 1g: 10 times the volume, adjust the pH to 3.75-3.85, put it on for 1 hour, filter with gauze, press dry, and take the precipitate. Dialysate [NaCl, Sodium Caprylate, Activated Carbon]→[pH5-5.2, 67-69℃]Filtrate[NaCl]→[pH3.75-3.85] Precipitate dialysis, adsorption precipitation, add 0.5 mol/L, 1.0 mol/L in turn , 2 mol/L sodium hydroxide, stir into a paste, adjust the pH to 7.8-8, wrap it in a cellophane dialysis bag, and dialyze against running water until the chloride ion concentration is not higher than the blank control of tap water. Combine the dialysate, treat with 2-4g/L aluminum hydroxide or 20-30 g/L activated carbon as adsorbent, stir for 1h, filter, and collect the filtrate. Precipitate [sodium hydroxide] → [vs. flowing water, pH 7.8-8] dialysate [aluminum hydroxide] → [1h] filtrate is clarified, sterilized, filtered to take the filtrate, adjust the pH to 6.6-7, and dilute the protein to 100g/L or 250g/L, sterilized and filtered, and then bottling or freeze-dried after passing the inspection. Filtrate [clarification, sterilization, filtration] → [pH 6.6-7] filtrate [sterile packaging] → finished albumin method 2. Human plasma is used as raw material for complexation and dissociation, and human serum is pumped into a stainless steel sandwich reaction tank Inside, turn on the stirrer, adjust the pH to 8.6 with sodium carbonate solution, add an equal volume of 2% rivanol, stir and stand for 2-4 hours, separate, and the supernatant is used to prepare human gamma globulin and collect the complex precipitate. Dilute the precipitate with sterile distilled water, adjust the pH to weak acidity, add sodium chloride, stir and dissociate, and use tap water interlayer to circulate and cool to obtain the dissociation liquid. Human plasma [rivanol, sodium carbonate]→[pH8.6] complex [distilled water, NaCl, HCl]→[weak acid, 65℃] dissociation liquid separation, sterilization dissociation liquid separation with basket centrifuge The centrifugal separation liquid is clarified and filtered with a pressure filter, and concentrated with an ultrafilter to obtain a concentrated liquid. The concentrated solution is treated at a constant temperature of 60°C for 10 hours, clarified and filtered with a pressure filter, sterilized by a cold sterilization system, and filled with albumin products after inspection. Dissociation solution [ultrafiltration] → [separation, concentration] concentrated solution [heat treatment] → [60℃, 10h] heat treatment solution [sterilization] → finished albumin

Uses

The main function is to maintain the plasma colloid osmotic pressure. It can be used as a blood volume expander to increase the colloid osmotic pressure and increase blood volume. Used to prevent or rescue hemorrhagic shock, traumatic shock, severe burns, scalds, etc. It can also supplement body albumin to treat hypoproteinemia, increased cerebral pressure caused by liver cirrhosis and kidney disease, and albumin deficiency in miscarriage or premature women. Shock caused by severe burns or blood loss due to other reasons. Note: When patients with nephropathy are administered intravenously, they should not be diluted with normal saline. Severe anemia, patients with heart failure should not be used in large quantities.

Drug Function and Efficacy

1. Increase blood volume and maintain plasma colloidal osmotic pressure: Albumin accounts for 80% of plasma colloidal osmotic pressure and mainly regulates the dynamic balance of water between tissues and blood vessels; 2. Transport and detoxification: Albumin can bind to both anions and cations, can transport different substances, and can also transport toxic substances to detoxification organs; 3. Nutritional supply: Tissue protein and plasma protein can be converted into each other. When nitrogen metabolism is impaired, albumin can serve as a nitrogen source to provide nutrition for tissues.
