Lactic acidosis
Lactic acid is reduced from pyruvate and is an intermediate metabolite of sugar. It is reduced to lactic acid when there is a lack of O2 or pyruvate is not oxidized. Acidosis caused by increased blood lactate levels due to various reasons is called lactic acidosis. The lactate concentration in the fasting venous blood (at rest) of a normal person is 0.4-1.4mmol/L, and the pyruvate concentration is 0.07-0.14mmol/L. The ratio of the two is 10:1, generally <15:1, and they are usually in a state of equilibrium; when the lactate concentration exceeds 2mmol/L (some believe it is >5mmol/L), HCO-3 ≤10mmol/L, lactate/pyruvate>10 and other causes of acidosis can be ruled out, this disease can be diagnosed.

Sodium Bicarbonate Injection
Indication: 1. Treatment of metabolic acidosis. For the treatment of mild to moderate metabolic acidosis, oral administration is preferred. Severe metabolic acidosis should be treated by intravenous drip, such as severe kidney disease, circulatory failure, cardiopulmonary resuscitation, extracorporeal circulation, severe primary lactic acidosis, diabetic ketoacidosis, etc. 2. Alkalize urine. Used for the prevention of uric acid kidney stones, reducing the nephrotoxicity of sulfonamide drugs, and acute hemolysis to prevent hemoglobin deposition in the renal tubules. 3. As an antacid, it treats symptoms caused by excessive gastric acid. 4. Intravenous drip has a non-specific therapeutic effect on certain drug poisonings, such as poisoning by barbiturates, salicylates and methanol. However, this product is contraindicated for gastric lavage when swallowed in case of strong acid poisoning, because this product reacts with strong acid to produce a large amount of carbon dioxide, which leads to acute gastric dilatation or even gastric rupture.
