Mastitis refers to acute mastitis, a disease name. Carbuncle occurs in the breast. It comes from Volume 5 of "Zhou Hou Jiu Zu Fang". It is also known as (疒乇)乳, 妬乳, 乳毒, 吹你, 吹乳, 内吹, 外吹, 乳根藈, 乳疯, etc. It is mostly caused by stagnation of liver qi, congestion of stomach heat, stasis of milk, and the gathering of wind and cold evil. "Zhu Bing Yuan Zheng Lun·Women's Miscellaneous Diseases" says: "There are also cases where milk is accumulated, fighting with blood, accumulating heat, and gathering to form mammary abscesses." At the beginning, the symptoms are that hard lumps of varying sizes appear in the breast, and the patient begins to feel distended and painful. The milk is not sucked smoothly and stagnant, and the patient gradually feels chills and fever, or headaches, and discomfort in the limbs. Then the lump will increase in size, become red and painful, and the fever and chills will not subside. In the worst case, the pain will intensify, the whole body will feel sore and weak, and the local lump will change from hard to soft, with a sense of fluctuation, indicating that pus has formed. In the early stage of the disease, the treatment should be to soothe the liver and regulate qi, activate blood and clear heat, and clear milk and disperse knots. It is more common in women after childbirth. The causes include liver qi stagnation, stomach heat stagnation; or milk stagnation; or nipple damage during breastfeeding, infection with heat toxins; or postpartum blood deficiency, feeling of external evil, resulting in damp heat accumulation, qi and blood stagnation. Western medicine believes that this disease is mostly caused by decreased resistance after childbirth, nipple damage, milk stasis, bacteria invading the breast along lymphatic vessels and milk ducts, and secondary infection. The pathogens are mostly Staphylococcus aureus, followed by Staphylococcus aureus and Escherichia coli. For internal treatment, you can choose Trichosanthes and Burdock Soup, or Juye Powder, or Heru Decoction with modifications; for external treatment, you should suck milk, cooperate with Ruyi Jinhuang Powder, or mash dandelion and apply it, or use moxibustion with garlic, or alternate moist heat external application, or hot ironing. If pus has formed, it should be surgically incised and drained as soon as possible, and the pus will be hot and the severe pain will stop immediately. If pus has not yet formed, you can choose Tuoli detoxification agent, such as Tuoli Tounong Decoction. It can also be treated according to the syndrome differentiation.
