Function and Efficacy

Sorbitol monosaccharide is an isomer of mannitol, and its effect is similar to that of mannitol but weaker. After intravenous injection of concentrated solution (25%) of this product, except for a small part converted into sugar, most of it is excreted through the kidney in its original form. Due to the formation of blood hypertonicity, it can dehydrate the surrounding tissues and brain parenchyma and be excreted from the urine along with the drug, thereby reducing intracranial pressure and eliminating edema. High efficacy appears 2 hours after injection, and the brain edema is gradually calmed down, the tension disappears, the cerebrospinal fluid pressure decreases, it is not metabolized in the body, and is rarely reabsorbed in the renal tubules after glomerular filtration, playing an osmotic diuretic role. 1 Tissue dehydration effect. Appearance

This product is a colorless clear liquid.

Indication

It is suitable for the treatment of cerebral edema and glaucoma, and can also be used for edema and oliguria in patients with normal heart and kidney function.

Usage and Dosage

Intravenous drip, 250-500 ml of 25% solution at a time, 1-2 g/kg for children at a time, infused within 20-30 minutes. To reduce cerebral edema, repeat the injection every 6-12 hours.

Adverse Reactions

1. Water and electrolyte disorders. The most common. (1) Rapid intravenous injection of large amounts of sorbitol can cause accumulation of sorbitol in the body, resulting in a rapid and large increase in blood volume, leading to heart failure (especially when there is heart damage), dilutional hyponatremia, and occasionally hyperemia. (2) Inappropriate excessive diuresis leads to a decrease in blood volume and aggravates oliguria. 2. Chills and fever. 3. Difficulty urinating. 4. Thrombophlebitis. 5. Sorbitol extravasation can cause tissue edema and skin necrosis. 6. Allergies cause rash, urticaria, dyspnea, and anaphylactic shock. 7. Dizziness and blurred vision. 8.

Precautions

1. Anuric patients diagnosed with acute tubular necrosis, including those who do not respond to sorbitol trials, because sorbitol accumulation causes increased blood volume and increases the burden on the heart; 2. Patients with severe water loss; 3. Patients with active intracranial bleeding, because volume expansion aggravates bleeding, except during intracranial surgery; 4. Acute pulmonary edema, or severe pulmonary congestion.

Special Population Medication

Precautions for the elderly: Elderly people are more likely to suffer from kidney damage when using this drug, and the chance of kidney damage increases with age.

Drug Interactions

1. It can increase the toxicity of digitalis and is related to hypokalemia. 2. It can increase the diuretic and intraocular pressure-lowering effects of diuretics and carbonic anhydrase inhibitors. The dose should be adjusted when combined with these drugs.
