Isosorbide dinitrate for injection

Function and Efficacy

The main pharmacological effect of isosorbide dinitrate (ISDN) is to relax vascular smooth muscle. ISDN is metabolized in the body to produce isosorbide mononitrate (ISMN), which releases nitric oxide (NO). NO is the same as endothelial relaxing factor, activating guanylate cyclase, increasing cyclic guanosine monophosphate (cGMP) in smooth muscle cells, thereby relaxing vascular smooth muscle, dilating peripheral arteries and veins, and having a stronger dilating effect on veins. Venous dilation causes blood to remain in the periphery, reducing the amount of blood returning to the heart, and reducing left ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload). Arterial dilation reduces peripheral vascular resistance, systolic arterial pressure and mean arterial pressure (afterload). Coronary artery dilation increases coronary perfusion. The overall effect is to reduce myocardial oxygen consumption and increase oxygen supply.

Ingredients

Isosorbide dinitrate

Name Description Content CAS NO. Manufacturer
Isosorbide dinitrateIngredients

It relaxes vascular smooth muscle, releases nitric oxide through metabolism to generate isosorbide mononitrate, activates guanylate cyclase, increases cyclic guanosine monophosphate in smooth muscle cells, and thus relaxes vascular smooth muscle. It dilates peripheral arteries and veins, reduces the amount of blood returning to the heart, reduces left ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload), reduces peripheral vascular resistance, systolic arterial pressure and mean arterial pressure (afterload), increases coronary artery perfusion, reduces myocardial oxygen consumption and increases oxygen supply.

Appearance

White or off-white freeze-dried mass or powder.

Indication

It is suitable for left ventricular failure secondary to acute myocardial infarction, left ventricular failure caused by various causes, and severe or unstable angina pectoris.

Usage and Dosage

The dosage needs to be adjusted according to the patient's response. The normal dosage is 2-7 mg of isosorbide dinitrate per hour, but it can be increased to 10 mg per hour if necessary. The starting dose of intravenous drip is 30 μg/min. Observe for 0.5-1 hour. If there is no adverse reaction, the dosage can be doubled. Once a day, 10 days as a course of treatment.

Adverse Reactions

1. Like other nitrate drugs, this product may cause symptoms such as headache, nausea and tachycardia during use; 2. Due to the significant decrease in systemic arterial pressure, it may cause cerebral insufficiency and reduced coronary perfusion, but clinical experience shows that the above situation is not common, which is consistent with the fact that this product has a stronger vasodilator effect on the venous system than on the arterial system.

Precautions

1. Patients with known allergy to nitrates; 2. Cardiogenic shock (unless diastolic blood pressure can be maintained appropriately, such as by combining drugs that increase myocardial contractility). 3. Patients with circulatory failure and severe hypotension; 4. Patients with significant anemia, head trauma, cerebral hemorrhage, severe hypotension or hypovolemia; 5. Patients who are taking sildenafil (VIAGRA) in combination, as sildenafil can significantly enhance the hypotensive effect of isosorbide dinitrate; 6. Patients with glaucoma.

Drug Interactions

This product may enhance the antihypertensive effect of other antihypertensive drugs; this product may enhance certain effects of alcohol. Taking nitrous oxide donors at the same time, such as the active nitric oxide in this product and sildenafil (VIAGRA) can significantly enhance the antihypertensive effect of this product.

Storage

Keep away from light, tightly closed, in a cool place.
