Epinephrine Hydrochloride Injection
Function and Efficacy
It has both α-receptor and β-receptor stimulating effects. Alpha-receptor stimulating causes vasoconstriction of the skin, mucous membranes, and visceral vessels. β-receptor stimulating causes coronary vasodilation, skeletal muscle, myocardial excitation, increased heart rate, and relaxation of bronchial smooth muscle and gastrointestinal smooth muscle. The effect on blood pressure is dose-related. Common doses increase systolic blood pressure while diastolic blood pressure does not increase or decreases slightly. High doses increase both systolic and diastolic blood pressure.

Indication
It is mainly used for severe breathing difficulties caused by bronchospasm, can quickly relieve anaphylactic shock caused by drugs, and can also be used to prolong the duration of action of infiltration anesthesia drugs. It is the main rescue drug for cardiopulmonary resuscitation of cardiac arrest caused by various reasons.
Usage and Dosage
Usual dosage: subcutaneous injection, 0.25mg-1mg per time; Maximum dosage: subcutaneous injection, 1mg per time.
Adverse Reactions
1. Palpitations, headaches, elevated blood pressure, tremors, weakness, dizziness, vomiting, cold limbs. 2. Arrhythmias may occur sometimes, and in severe cases, ventricular fibrillation may cause death. 3. Edema, congestion, and inflammation may occur at the site of medication.
Precautions
1 Use with caution in the following situations: organic encephalopathy, cardiovascular disease, glaucoma, Parkinson's disease, circulatory collapse and hypotension caused by thiazides, and mental and neurological diseases. 2 Excessive dosage or accidental injection into blood vessels during subcutaneous injection may cause a sudden rise in blood pressure and lead to cerebral hemorrhage. 3 The dose of local anesthesia should not exceed 300ug each time, otherwise it may cause palpitations, headaches, and increased blood pressure. 4 There are cross-allergic reactions with other sympathomimetics. 5 It can pass through the placenta. 6 When treating allergic shock, blood volume must be supplemented.
Special Population Medication
Precautions for children: Use with caution when this product must be used. Precautions for pregnancy and lactation: Use with caution when this product must be used. Precautions for the elderly: The elderly are sensitive to sympathomimetic drugs and should be cautious when this product must be used.
Drug Interactions
1α-receptor blockers and various vasodilators can counteract the pressor effect of this product. 2 When used in combination with general anesthetics, it is easy to produce arrhythmias and even ventricular fibrillation. When used for local anesthesia of fingers and toes, this product should not be added to the solution to avoid insufficient blood supply to the extremities and necrosis. 3 When used in combination with digitalis and tricyclic antidepressants, it can cause arrhythmias. 4 When used in combination with ergot preparations, it can cause severe hypertension and tissue ischemia. 5 When used in combination with reserpine and guanethidine, it can cause hypertension and tachycardia. 6 When used in combination with beta-receptor blockers, the beta receptor effects of the two cancel each other out, which may cause abnormally high blood pressure, bradycardia and bronchial constriction. 7 When used in combination with other sympathomimetic amines, the cardiovascular effect is aggravated and side effects are prone to occur. 8 When used in combination with nitrates, the pressor effect of this product is offset and the anti-anginal effect of nitrates is weakened.
