Function and Efficacy
This product (i.e., this compound, the same below) is a compound preparation composed of piperacillin sodium and sulbactam sodium in a ratio of 4:1. Piperacillin is a semi-synthetic penicillin that inhibits the synthesis of bacterial cell walls by binding to bacterial penicillin-binding proteins (PBPs) to kill bacteria. It has the characteristics of broad spectrum and low toxicity, but is easily hydrolyzed by beta-lactamase produced by bacteria to produce drug resistance. Sulbactam is a beta-lactamase inhibitor that can inhibit type II-V beta-lactamase and is a competitive, irreversible inhibitor. The combined use of piperacillin and sulbactam enhances the antibacterial activity of piperacillin, expands the antibacterial spectrum, and enhances its antibacterial activity against piperacillin-resistant enzyme-producing bacteria.

Appearance
This product is white or off-white powder or crystalline powder.
Indication
It is suitable for the following infections caused by β-lactamase-producing pathogens that are resistant to piperacillin and sensitive to this product. When used to treat mixed infections caused by bacteria that are sensitive to piperacillin alone and β-lactamase-producing bacteria that are resistant to piperacillin alone and sensitive to this product, no other antibiotics are needed. 1. Respiratory system infections: including acute bronchitis, pneumonia, acute exacerbation of chronic bronchitis, bronchiectasis and concurrent infection, etc. 2. Urinary system infections: including simple urinary system infections and complicated urinary system infections.
Usage and Dosage
Adults: 2.5g or 5g (i.e., 2g or 4g of piperacillin, 0.5g or 1g of sulbactam) every 12 hours. For severe or refractory infections, 2.5g or 5g (i.e., 2g or 4g of piperacillin, 0.5g or 1g of sulbactam) every 8 hours. For patients with renal insufficiency, the dose should be adjusted as appropriate. Treatment course: 7-14 days, or the course of treatment can be adjusted according to the condition.
Adverse Reactions
1. Common adverse reactions of this product include: 1) Skin reactions: such as rash, spasmodic itching, etc.; 2) Digestive tract reactions: such as diarrhea, nausea, vomiting, etc.; 3) Allergic reactions; 4) Local reactions: such as local irritation reaction after injection, pain, phlebitis, thrombophlebitis and edema, etc.; 5) Other reactions: such as thrombocytopenia, pancreatitis, fever, fever with eosinophilia, diarrhea or elevated transaminase, etc. 2. In addition, the following adverse reactions can also be seen with this product: 1) Diarrhea, constipation, nausea, vomiting, abdominal pain, indigestion, etc.; 2) Maculopapular rash, scar rash, urticaria, eczema, itching, etc.; 3) Irritability, dizziness, anxiety, etc., 4) Other reactions: such as rhinitis, dyspnea, etc.
Precautions
It is contraindicated in patients with a history of allergy to penicillins or a positive penicillin skin test.
Drug Interactions
1. When this product is used in combination with probenecid, it can reduce the renal clearance of this product and prolong its half-life. 2. When this product is used simultaneously with tobramycin, it can reduce the area under the curve and renal clearance of tobramycin. 3. The activity of aminoglycoside antibiotics can be reduced due to the presence of penicillins. 4. When piperacillin is used simultaneously with the non-polar muscle relaxant vecuronium bromide, the neuromuscular blocking effect of vecuronium bromide can be prolonged. 5. When piperacillin is taken simultaneously with heparin, oral anticoagulants and other drugs that may affect the blood coagulation system and platelet function, coagulation indicators should be monitored regularly.
