Clindamycin Phosphate Injection
Function and Efficacy
1. This product is a chemically semi-synthetic clindamycin derivative. It has no antibacterial activity in vitro. After entering the body, it is rapidly hydrolyzed into clindamycin and shows its pharmacological activity. Therefore, the antibacterial spectrum, antibacterial activity and therapeutic effect are the same as those of clindamycin, but its fat solubility and permeability are better than those of clindamycin, and it can be administered by intramuscular injection and intravenous drip. 2. Compared with lincomycin, this product has a 4-8 times stronger antibacterial effect, good absorption, high bone concentration, and good efficacy against anaerobic infections. 3. This product has strong antibacterial activity mainly against Gram-positive cocci and anaerobic bacteria, including Gram-positive cocci: Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus (except Streptococcus phosphotrophicus), Streptococcus pneumoniae, Micrococcus, etc.; anaerobic bacteria: Clostridium, Bacteroides, Clostridium, Propionibacterium, Eubacterium, anaerobic cocci, etc.

Appearance
This product is a colorless or slightly yellow clear liquid.
Indication
Used for the following infectious diseases caused by Gram-positive bacteria: 1. Tonsillitis, suppurative otitis media, sinusitis, etc. 2. Acute bronchitis, acute exacerbation of chronic bronchitis, pneumonia, lung abscess and bronchiectasis combined infection, etc. 3. Skin and soft tissue infections: furuncle, carbuncle, abscess, cellulitis, trauma, burns and postoperative infection, etc. 4. Urinary tract infections: acute urethritis, acute pyelonephritis, prostatitis, etc. 5. Others: osteomyelitis, sepsis, peritonitis and oral infection, etc. Used for various infectious diseases caused by anaerobic bacteria: 1. Empyema, lung abscess, anaerobic pneumonia. 2. Usage and Dosage
This product can be administered by deep intramuscular injection or intravenous drip. When intravenously dripping, every 0.3g needs to be diluted with 50-100ml of normal saline or 5% glucose solution to a concentration of less than 6mg/ml, and drip slowly, usually not more than 20mg per minute. 1. Mild to moderate infection: Adults: 0.6-1.2g per day, divided into 2-4 doses (q12h-q6h), children: 15-25mg/kg per day according to body weight, divided into 2-4 doses (q12h-q6h). 2. Severe infection: Adults: 1.2-2.7g per day, divided into 2-4 doses (q12h-q6h), children: 25-40mg/kg per day according to body weight, divided into 2-4 doses (q12h-q6h).
Adverse Reactions
1. After intramuscular injection, slight pain may occasionally occur at the injection site. Long-term intravenous infusion may cause phlebitis. 2. Gastrointestinal reactions: nausea, vomiting, abdominal pain and diarrhea may occur occasionally. 3. Allergic reactions: a few patients may develop drug-induced rash. 4. Occasionally, it may cause neutropenia or eosinophilia. 5. A few patients may experience transient alkaline phosphatase, mild elevation of serum chlorotransferase and jaundice. 6. A very small number of patients may develop pseudomembranous colitis.
Precautions
This product has cross-resistance with lincomycin and clindamycin and is contraindicated in patients with a history of allergy to clindamycin or lincomycin.
Special Population Medication
Precautions for children: Use with caution in children under 4 years old. Precautions for pregnancy and lactation: No studies have been conducted on pregnant women, but no effects on the fetus have been seen in animal reproduction studies, and the therapeutic benefits of using this drug in pregnant women may outweigh its potential harm. Alternatively, this drug has not been tested on animals, and no sufficiently rigorous controlled studies have been conducted on pregnant women. Pregnant and lactating women should be careful when using this product. Precautions for the elderly: 1. Pharmacokinetic studies of clindamycin have shown that there is no significant difference in pharmacokinetics between young patients with normal liver and kidney function and elderly patients after oral or intravenous injection of clindamycin. 2. Clinical studies of clindamycin have not included a sufficient number of patients aged 65 and over, and it is currently difficult to determine whether the clinical response of the elderly is significantly different from that of young patients. However, clinical experience suggests that antibiotic-related colitis and diarrhea (caused by Clostridium difficile) occur more frequently in the elderly (>60 years old) and are more severe. Therefore, when the elderly use this product, they should pay attention to careful observation or monitoring of diarrhea in these people.
Drug Interactions
This product is antagonistic to erythromycin and should not be used together.
