Ibandronic acid injection

Function and Efficacy

This drug is a bisphosphonate bone resorption inhibitor. Since bisphosphonates have a high affinity for inorganic substances in bones, they can specifically act on bone tissue. Its exact mechanism of action is still unclear, but it may mainly inhibit the dissolution and formation of hydroxyapatite by binding to the bone, thereby producing an anti-bone resorption effect. In addition, the pharmacological action of this drug may also be related to directly changing the morphology of osteoclasts or directly inhibiting cytokines mediated by osteoblasts. Animal experiments have shown that the anti-bone resorption effect of this drug is stronger than that of clodronate and pamidronate, which are 500 times and 50 times stronger than the latter two, respectively.

Indication

Pathological (abnormal) increase in blood calcium (hypercalcemia) caused by a tumor.

Usage and Dosage

Adults·Regular dose·Intravenous drip The dosage should be determined according to the severity of hypercalcemia and the type of tumor. For most patients with severe hypercalcemia (blood calcium concentration corrected by albumin is greater than or equal to 3mmol/L or 12mg/dl), a single dose of 4mg is effective; for patients with moderate hypercalcemia (blood calcium concentration corrected by albumin is less than 3mmol/L or 12mg/dl), a single dose of 2mg is effective. In clinical trials, the highest single dose of this drug was 6mg, but this dose did not further improve the efficacy. The formula for calculating the serum calcium concentration corrected by albumin is: serum calcium concentration corrected by albumin (mmol/L) = serum calcium concentration (mmol/L) - [0.02×albumin (g/L)] + 0.8 serum calcium concentration corrected by albumin (mg/dl) = serum calcium concentration (mg/dl) + 0.8×[4-albumin (g/dl)] [Foreign Usage and Dosage Reference] Adults · Conventional dose · Oral administration 1. Prevention and treatment of osteoporosis: The recommended dose is 2.5 mg per day, and 1000 mg of calcium is taken daily, 1 hour before meals, for at least 1 year of treatment; or, 20 mg once a week, for at least 1 year of treatment. 2. Prevention of skeletal events and bone metastasis in breast cancer patients: 50 mg once a day, 1 half an hour before meals. · Intravenous drip 1. Hypercalcemia: For patients with moderate hypercalcemia, the recommended dose is 2 mg per dose, and the drip time should exceed 2 hours. For patients with severe hypercalcemia, the recommended dose is 4-6 mg per dose. 2. Prevention and treatment of osteoporosis: The recommended dose is 2 mg per day, once every 3 months, for at least 1 year. 3. Prevention of skeletal events and bone metastasis in breast cancer patients: 6 mg once, once every 3-4 weeks.

Adverse Reactions

Fever most commonly occurs after intravenous infusion of Bonrolix. Individual reports of a flu-like syndrome including fever, chills, bone and/or muscle pain have occurred. In most cases, these symptoms disappear within hours or days and do not require special treatment. Decreased renal calcium excretion is usually accompanied by a decrease in serum phosphorus levels and generally does not require treatment. Blood calcium concentrations may fall below normal levels. Intolerable gastrointestinal reactions (including gastric and small intestinal side effects) have occurred in individual cases. Asthma patients who are sensitive to acetylsalicylic acid may induce bronchospasm (wheezing, dyspnea) when receiving other bisphosphonate treatments. If other reactions beyond the therapeutic effect occur, especially side effects not listed in the drug instructions, the patient should promptly notify the doctor or pharmacologist.

Precautions

It is contraindicated for patients who are allergic to the drug ingredients of Bonrolix and those with severe kidney disease (such as renal insufficiency, blood creatinine 5mg/dL, or 442umol/L). Patients who are allergic to other bisphosphonates should use Bonrolix with caution. Pregnant and lactating women should not receive Bonrolix treatment because there is currently a lack of data on reproductive toxicity tests and clinical experience with human pregnancy. Bonrolix cannot be used in children because of the lack of clinical experience in this area.

Drug Interactions

The interaction between ibandronic acid and other drugs has not been studied, and no interaction between Bonrolix and other drugs has been observed. Bonrolix should be used with caution when used simultaneously with aminoglycosides, as both can cause delayed hypocalcemia. Bonrolix should be used with caution in patients with hypomagnesemia.
