Function and Efficacy

Pharmacological action: The main ingredients of this compound are cefoperazone and tazobactam. Cefoperazone is a third-generation cephalosporin that achieves its bactericidal effect by inhibiting the biosynthesis of the cell wall of sensitive bacteria. Tazobactam has no antibacterial activity against other bacteria except Neisseriaceae and Acinetobacter, but it has an irreversible inhibitory effect on most important β-lactamases produced by β-lactam antibiotic-resistant strains. Therefore, tazobactam can prevent the destruction of penicillins and cephalosporins by related resistant bacteria, reflecting that tazobactam has a certain synergistic effect with penicillins and cephalosporins. Indication

It is only used to treat moderate to severe infections caused by β-lactamase-producing bacteria that are resistant to cefoperazone alone and sensitive to this product. When used to treat mixed infections caused by bacteria that are sensitive to cefoperazone alone and β-lactamase-producing bacteria that are resistant to cefoperazone alone and sensitive to this product, no other antibiotics are needed. Lower respiratory tract infections: pneumonia, acute exacerbation of chronic bronchitis, acute bronchitis, lung abscesses and other lung infections caused by sensitive bacteria such as β-lactamase-producing Pseudomonas aeruginosa, Streptococcus pneumoniae and other streptococci, Klebsiella pneumoniae and other Klebsiella species, Haemophilus influenzae, Staphylococcus aureus, etc. Urogenital system infections: infections caused by β-lactamase-producing Escherichia coli, Proteus, Klebsiella species, Pseudomonas aeruginosa

Usage and Dosage

Intravenous drip. First dissolve with an appropriate amount of sodium chloride injection or sterile water for injection (5-10ml), then add 5% glucose injection or sodium chloride injection 150-250ml to dilute for intravenous drip, the drip time is 30-60 minutes, and each drip time must not be less than 30 minutes. The course of treatment is generally 7-10 days (severe infection can be appropriately extended). Adult dosage: 1.125-2.25g/time, intravenous drip once every 8 hours or 12 hours. For patients with severe renal insufficiency (creatinine elimination rate <30ml/min), the dose of tazobactam should not exceed 0.5g every 12 hours.

Adverse Reactions

Patients generally tolerate this drug well, and most adverse reactions are mild and disappear after drug discontinuation. (1) Gastrointestinal: Like other β-lactam antibiotics, common side effects of this drug are gastrointestinal reactions; the most common are loose stools and diarrhea, followed by nausea and vomiting. (2) Skin reactions: This drug may cause allergic reactions, manifested as maculopapular rash, urticaria, eosinophilia, and drug fever. (3) Blood: Long-term use of this drug may lead to reversible neutropenia, thrombocytopenia, prolonged prothrombin time, and decreased prothrombin activity, which may be seen in individual cases. Bleeding is rare and can be prevented and controlled with vitamin K. (4) Laboratory abnormalities: A few cases have aspartate transaminase.

Precautions

It is contraindicated for patients who are allergic to any ingredient of this product or other β-lactam antibiotics.

Special Population Medication

Precautions for children: The safety and effectiveness of the drug for children are not yet clear. When it must be used, the pros and cons should be weighed. Precautions for pregnancy and lactation: Animal experiments have not found that this product harms reproductive ability and fetuses, and there is still a lack of sufficient controlled research data in humans. Therefore, pregnant women should use it with caution. Cefoperazone and tazobactam can be secreted into breast milk in small amounts, so lactating women should use it with caution. Precautions for the elderly: The elderly have physiological liver and kidney function impairment, so this product should be used with caution and the dosage needs to be adjusted.

Drug Interactions

1 When used in combination with aminoglycoside antibiotics (gentamicin and tobramycin), it has a synergistic effect against certain sensitive strains of Enterobacteriaceae and Pseudomonas aeruginosa. However, there are physical incompatibility between this product and aminoglycoside antibiotics, so the two liquids cannot be directly mixed. If combined use is required, the two drugs can be injected intravenously in sequence. Different intravenous infusion lines should be used for injection, or the previously used intravenous infusion line should be fully flushed with another approved diluent between injections.
