Cisatracurium Besylate for Injection
Function and Efficacy
Pharmacodynamic properties Cisatracurium besylate is a neuromuscular blocker, ATC code: M03AC11. Cisatracurium besylate is a moderately effective, non-depolarizing skeletal muscle relaxant with an isoquinolinium benzyl ester structure. Human clinical studies have shown that this product is not associated with dose-dependent histamine release, even at doses as high as 8 times the ED95. Mechanism of action Cisatracurium besylate binds to cholinergic receptors on the motor end plate to antagonize the effects of acetylcholine, thereby producing a competitive neuromuscular blockade. This effect is easily antagonized by anticholinesterase drugs such as neostigmine or edrophonium. When anesthetized with opioids (sodium thiopental/fentanyl/midazolam), the ED95 (the amount of drug required to inhibit the twitch response of the adductor pollicis muscle induced by stimulation of the ulnar nerve by 95%) of cisatracurium is approximately 0.05 mg/kg body weight. Under halothane anesthesia, the ED95 of cisatracurium in children is 0.04 mg/kg body weight.

Appearance
This product is off-white or slightly yellow freeze-dried block or powder
Indication
This product is used for surgery and other operations as well as intensive care treatment. As an adjunct to general anesthesia or as a sedative in the intensive care unit (ICU), it can relax skeletal muscles and make endotracheal intubation and mechanical ventilation easier.
Usage and Dosage
Cisatracurium besylate is only allowed to be used for intravenous injection or drip. The drug should be dissolved in normal saline for injection before use. The dosage of cisatracurium besylate must be individualized. In clinical practice, the use of peripheral nerve stimulation can achieve the best application of cisatracurium besylate, reduce the possibility of drug overdose or underdose, and help evaluate the patient's recovery.
Adverse Reactions
Adverse reactions to the use of this product have been recorded as skin flushing or rash, bradycardia, hypotension and bronchospasm. Allergic reactions of varying degrees can be observed after the use of neuromuscular blockers. In rare cases, severe allergic reactions have been reported when this product is used in combination with one or more anesthetics. There have been reports of muscle weakness and/or myopathy after prolonged use of muscle relaxants in seriously ill patients in the intensive care unit. Most patients also receive steroid preparations, and the above situation has occasionally been reported after the use of this product, but the causal relationship has not yet been determined.
Precautions
This product is contraindicated in patients with known hypersensitivity to cisatracurium, atracurium or benzenesulfonic acid or to any of the ingredients in this product.
Drug Interactions
Many drugs can affect the intensity and/or duration of action of non-depolarizing neuromuscular blocking drugs, including: Drugs that enhance efficacy: Anesthetics: such as enflurane, isoflurane and halothane (see [Dosage and Administration]), ketamine. Other non-depolarizing neuromuscular blocking drugs. Other drugs: Antibiotics: including aminoglycosides, polymyxins, spectinomycin, tetracycline, lincomycin and clindamycin. Antiarrhythmic drugs: including propranolol, calcium channel blockers, lidocaine, procainamide and quinidine. Diuretics: including furosemide, and may also include thiazides, mannitol and acetazolamide. Magnesium salts. Lithium salts. Ganglionic blockers: trimethiazide, hexamethonium. Drugs that reduce efficacy: For patients who have been on phenytoin and carbamazepine for a long time, the efficacy of this product may be reduced. Prior administration of succinylcholine does not affect the duration of neuromuscular blockade following intravenous injection or infusion of this product, and there is no need to change the single dose or infusion rate. The co-administration of succinylcholine to prolong the effect of non-depolarizing neuromuscular blocking agents may result in a prolonged combined blockade that is difficult to reverse with anticholinesterase drugs. Rarely, certain drugs can exacerbate or induce latent myasthenia gravis attacks, or actually induce a myasthenic syndrome; in this case, the patient's sensitivity to non-depolarizing neuromuscular blocking agents may be increased. These drugs include various antibiotics, beta-blockers (propranolol, propranolol), antiarrhythmics (procainamide, quinidine), antirheumatic drugs (chloroquine, D-penicillamine), trimetipraz, chlorpromazine, steroids, phenytoin, and lithium.
