Propranolol Hydrochloride Tablets

Function and Efficacy

1. Propranolol is a non-selective competitive inhibitor of adrenergic β-receptor blockers. It blocks β1 and β2 receptors on the heart, antagonizes sympathetic nerve excitation and catecholamine effects, reduces the contractility and contraction speed of the heart, inhibits vascular smooth muscle contraction, reduces myocardial oxygen consumption, and restores the oxygen supply and demand relationship of ischemic myocardium to a low level. It can be used to treat angina pectoris. 2. Inhibits adrenergic excitement of cardiac pacemaker potentials and is used to treat arrhythmias. This product can also be used to treat hypertension by blocking central and adrenergic neurons, inhibiting renin release and reducing cardiac output. 3. Competitively antagonizes the effects of isoproterenol and norepinephrine, blocks β2 receptors, and reduces plasma

1. It is a non-selective competitive inhibitor of adrenergic β-receptor blocker, blocking β1 and β2 receptors on the heart, antagonizing sympathetic nerve excitation and catecholamine effects, reducing the contractile force and contractile speed of the heart, while inhibiting vascular smooth muscle contraction, reducing myocardial oxygen consumption, and restoring the oxygen supply and demand relationship of ischemic myocardium to a low level of balance, which can be used to treat angina pectoris. 2. It inhibits the adrenergic excitement of cardiac pacemaker potentials and is used to treat arrhythmias. 3. It can be used to treat hypertension by blocking central and adrenergic neurons, inhibiting renin release and reducing cardiac output. 4. It competitively antagonizes the effects of isoproterenol and norepinephrine, blocks β2 receptors, and reduces plasma-related activity.

Indication

1 Hypertension (alone or in combination with other antihypertensive drugs). 2 Exertional angina pectoris. 3 Control supraventricular tachyarrhythmias and ventricular arrhythmias, especially those related to catecholamines or caused by digitalis. It can be used to control the ventricular rate of atrial flutter and atrial fibrillation that are not well treated with digitalis, and can also be used for refractory premature contractions to improve patients' symptoms. 4 Reduce the outflow tract pressure difference in hypertrophic cardiomyopathy, relieve symptoms such as angina pectoris, palpitations and syncope. 5 Used in combination with alpha-receptor blockers to control tachycardia in patients with pheochromocytoma. 6 Used to control the rapid heart rate of hyperthyroidism, and can also be used to treat thyroid

Usage and Dosage

1 Hypertension: Oral, initial dose 10mg, 3-4 times a day, can be used alone or in combination with diuretics. The dose should be gradually increased, the maximum daily dose is 200mg. 2 Angina pectoris: 5-10mg at the beginning, 3-4 times a day; increase 10-20mg every 3 days, can gradually increase to 200mg per day, divided into several doses. 3 Arrhythmia: 10-30mg per day, 3-4 times a day. Take before meals and before bedtime. 4 Myocardial infarction: 30-240mg per day, 2-3 times a day. 5 Hypertrophic cardiomyopathy: 10-20mg, 3-4 times a day. Adjust the dose according to needs and tolerance. 6

Adverse Reactions

The use of this product may cause adverse reactions of the central nervous system such as dizziness, confusion (especially in the elderly), depression, slow reaction, etc.; dizziness (caused by hypotension); slow heart rate (<50 beats/min); less common are bronchospasm and dyspnea, congestive heart failure; even rarer are fever and sore throat (granulocytopenia), rash (allergic reaction), bleeding tendency (thrombocytopenia); when adverse reactions persist, be particularly vigilant against Raynaud's sign such as cold limbs, diarrhea, fatigue, dry eyes, mouth or skin, nausea, numbness of fingers and toes, abnormal fatigue, etc.

Precautions

This product is contraindicated in the following cases: 1. Patients with Ⅱ and Ⅲ degree heart block. 2. Patients with cardiogenic shock. 3. Patients with a history of bronchospasm. 4. Patients after long-term fasting. 5. Metabolic acidosis (e.g. some diabetic patients). 6. Severe heart failure. 7. Sinus bradycardia.

Special Population Medication

Precautions for children: Not yet determined, generally 0.5~1.0mg/kg per day by body weight, taken orally in divided doses. The therapeutic range of blood drug concentration of this product is similar to that of adults when the dosage for children is calculated based on body weight. However, the therapeutic range of blood drug concentration of this product is higher than that of adults when the dosage for children is calculated based on body surface area. It has been reported that when patients with congenital idiocy take this product, the blood drug concentration increases, thereby improving bioavailability. Precautions for pregnancy and lactation: This product can enter the fetus through the placenta. It has been reported that it can cause intrauterine fetal growth retardation and dystocia caused by weakness during delivery after use in pregnant women with hypertension. Newborns may have hypotension, hypoglycemia, respiratory depression, and slow heart rate. Although it has been reported that it has no effect on the mother and the fetus, it must be used with caution and should not be used as the first-line treatment for pregnant women. This product can be secreted in small amounts from breast milk, so it should be used with caution in lactating women. Precautions for the elderly: Because elderly patients have low ability to metabolize and excrete drugs, the dosage should be adjusted appropriately when using this product.

Drug Interactions

1 Interaction with antihypertensive drugs: When used in combination with reserpine, this product can cause postural hypotension, bradycardia, dizziness, and syncope. When used in combination with monoamine oxidase inhibitors, it can cause extreme hypotension. 2 When used in combination with digitalis, atrioventricular block may occur and the heart rate may slow down, so close observation is required. 3 When used in combination with calcium antagonists, especially intravenous verapamil, be very vigilant about the inhibition of this product on the myocardium and conduction system. 4 When used in combination with epinephrine, phenylephrine, or sympathomimetic amines, it can cause significant hypertension, bradycardia, and atrioventricular block. 5 When used in combination with isoproterenol or xanthine, the latter's efficacy may be weakened. <br/
