After entering the body, organophosphate insecticides (such as dichlorvos, 1609, 1059, etc.) combine with cholinesterase in the body to form phosphorylase, which makes it lose the function of hydrolyzing acetylcholine, so acetylcholine accumulates in the body and a series of poisoning symptoms appear. Antidotes such as iodine pralidoxime can combine with the phosphoryl group in phosphorylated cholinesterase in the body, free the cholinesterase, and restore its activity of hydrolyzing acetylcholine, so it is also called cholinesterase resurrection agent. However, it is only effective for phosphorylated cholinesterase that has been formed not long ago, and the activity of the aged enzyme is difficult to restore, so the sooner the medication is used, the better. The characteristic of the action is that it can quickly eliminate muscle tremors and spasms, but it has a poor effect on eliminating salivation and sweating. Iodine pralidoxime and others can also directly combine with organophosphates in the blood to become non-toxic substances and be excreted in the urine.

It cannot counteract the effects of acetylcholine that has accumulated in the body, so it should be used in combination with atropine. (1) Treatment of mild poisoning: 0.4g/time for adults, diluted with glucose solution or saline, then dripped or slowly injected, repeated once every 2-4 hours if necessary. 15mg/kg for children. (2) Treatment of moderate poisoning: 0.8-1.2g for adults for the first time, 0.4-0.8g for the next 2 hours, for a total of 2-3 times; or maintained by intravenous drip, 0.4g every hour, for a total of 4-6 times. 20-30mg/kg for children. (3) Treatment of severe poisoning: 1-1.2g for adults for the first time, if ineffective after 30 minutes, 0.8-1.2g can be given, and 0.4g/time every hour thereafter. 30mg/kg for children once, dripped or slowly injected.

The therapeutic dose has little adverse reaction, but excessive single dose or too fast injection may cause dizziness, tachycardia, headache, convulsion, nausea, vomiting, etc. This product contains iodine, which may cause sore throat and parotid swelling. It is contraindicated for people who are allergic to iodine. It is easy to hydrolyze to form cyanide under alkaline conditions, so it should not be used in combination with alkaline drugs.
