Function and Efficacy

1. Positive inotropic effect at therapeutic doses: This product selectively binds to the Na--K ATPase of the myocardial cell membrane and inhibits the activity of the enzyme, which impairs the active coupled transport of Na-K inside and outside the myocardial cell membrane, increases the concentration of Na in myocardial cells, and thus activates the NaCa2 exchange on the sarcolemma, increases the amount of Ca2 in the cytoplasm, and increases the Ca2 reserve in the sarcoplasmic reticulum. When the myocardium is excited, more Ca2 is released; the increase in the concentration of Ca2 in myocardial cells excites myocardial contractile proteins, thereby increasing myocardial contractility. 2. Negative frequency effect: Due to its positive inotropic effect, it increases the cardiac output of the failing heart, improves the hemodynamic state, and eliminates the reflex increase in sympathetic nerve tension.

This product is a colorless clear liquid

Indication

1 Mainly used for heart failure. Due to its rapid action, it is suitable for patients with acute heart failure or acute exacerbation of chronic heart failure. 1 It can also be used to control the ventricular rate of patients with atrial fibrillation and atrial flutter accompanied by rapid ventricular rate. (3) It is slow to take effect in terminating supraventricular tachycardia and is rarely used.

Usage and Dosage

Common dosage for intravenous injection in adults: dilute with 5% glucose injection and inject slowly, the first dose is 0.4-0.6 mg, and then 0.2-0.4 mg can be given every 2-4 hours, with a total of 1-1.6 mg. Common dosage for children: divided into 2-3 doses with an interval of 3-4 hours according to the following doses. For premature infants and full-term newborns or children with renal impairment and myocarditis, intramuscular or intravenous injection is 0.022 mg/kg of body weight, and for 2 weeks to 3 years old, it is 0.025 mg/kg of body weight. After satisfactory therapeutic effect is obtained by intravenous injection of this product, the commonly used maintenance dose of digoxin can be used to maintain the therapeutic effect.

Adverse Reactions

1 Common adverse reactions include: new arrhythmias, poor appetite or nausea, vomiting (stimulation of the medullary center), lower abdominal pain, obvious weakness, and tenderness. 2 Rare reactions include: blurred vision or yellow vision (symptoms of poisoning), diarrhea, and central nervous system reactions such as depression or confusion. 3 Rare reactions include: drowsiness, headache, rash, urticaria (allergic reaction), etc. 4 Among the manifestations of poisoning, arrhythmias are the most important, and the most common is ventricular premature beats, which account for about 33% of adverse cardiac reactions. The second most common are atrioventricular block, paroxysmal or accelerated junctional tachycardia, paroxysmal atrial tachycardia with atrioventricular block, ventricular tachycardia, and ventricular fibrillation.

Precautions

Contraindications: 1. Use in combination with calcium injection; 2. Poisoning by any cardiac glycoside preparation; 3. Ventricular tachycardia, ventricular fibrillation; 4. Obstructive hypertrophic cardiomyopathy (can still be considered if accompanied by systolic dysfunction or atrial fibrillation); 5. Preexcitation syndrome with atrial fibrillation or flutter.

Drug Interactions

1. When used together with amphotericin B, corticosteroids or potassium-losing diuretics such as bumetanide (the product is buturamine) and ethacrynic acid (ethacrynic acid), it may cause hypokalemia and lead to digitalis poisoning. 2. When used together with antiarrhythmic drugs, calcium salt injections, cocaine, pancuronium bromide (Pancuronium Bromide, Pan Kelong, Bahuolang), rauwolfia alkaloids, succinylcholine (Scoline; Suxamethonium Chloride) or adrenergic drugs, it may cause arrhythmias due to additive effects. 3. Severe or complete atrioventricular conduction disorder with normal blood potassium
