Procaine epinephrine injection
Function and Efficacy
The main ingredient of this product, procaine hydrochloride, is a short-acting ester anesthetic. It is not suitable for surface anesthesia because of its weak penetration into the skin and mucous membranes. Local injection can penetrate the peripheral nerve membrane, block nerve conduction and produce anesthesia. Procaine in small doses can excite the sympathetic nerves, increase heart rate and blood pressure, and has no significant effect on cardiac output. Increasing the dosage can significantly inhibit myocardial contractility and reduce stroke volume. The increased concentration of the drug in the blood can inhibit atrioventricular conduction and bundle branch conduction. Procaine has a significant direct dilation effect on peripheral blood vessels, is easily absorbed into the blood, and the duration of anesthesia is short. In order to reduce absorption and prolong the efficacy and reduce toxic side effects, adding a small amount of epinephrine (1:200000-500000) can prolong the efficacy by 20%.
Indication
Local anesthetic. Used for infiltration anesthesia, block anesthesia and blockade therapy, etc.
Usage and Dosage
For local injection, the dosage is calculated based on procaine hydrochloride, and the dosage is as follows according to the purpose: Infiltration anesthesia and blockade therapy: 0.25%-0.5% solution is generally used for a larger injection range, and 1% solution is used for a smaller injection range. The amount of adrenaline added to each milliliter of the solution is generally 0.002-0.004mg, and the total amount shall not exceed 0.5mg. The dosage of procaine hydrochloride shall not exceed 1g each time. Block anesthesia: 1%-2% solution, the dosage shall not exceed 1g each time. Adult prescription limit: The single dosage shall not exceed 1g based on procaine hydrochloride.
Adverse Reactions
This product may cause hypersensitivity reactions and allergic reactions, and some patients may develop methemoglobinemia; once the blood concentration is high or it enters the blood vessels by mistake, it may cause a series of central nervous system and cardiovascular system poisoning reactions, such as convulsions, slow heart rate, and decreased blood pressure.
Precautions
1. It is contraindicated for patients who are allergic to this product or those with hypertension. 2. It is contraindicated for finger and toe block anesthesia.
Drug Interactions
1 This product can enhance the effect of muscle relaxants and prolong the duration of muscle relaxant effect. When used in combination with muscle relaxants, the dosage of muscle relaxants should be reduced. 2 This product can weaken the efficacy of sulfonamides and should not be used at the same time. 3 This product can enhance the effect of digitalis drugs and combined use can lead to their toxic reactions. 4 Anticholinesterase drugs such as neostigmine can interfere with the metabolism of this product and increase its toxicity. 5 This product can deepen the respiratory depression and hypotension caused by narcotic analgesics. 6 This product should not be used in combination with the following drugs: sodium bicarbonate, barbiturates, aminophylline, magnesium sulfate, heparin sodium, sodium nitroprusside, mannitol, neostigmine methylsulfate, hydrocortisone, dexamethasone, etc.
