Indication
It is suitable for ovarian cancer and breast cancer, and also has certain effects on lung cancer, colorectal cancer, melanoma, head and neck cancer, lymphoma, and brain tumors.
Usage and Dosage
1. First ask the patient whether he has a history of allergies, and check the data of white blood cells and platelets. Patients with a history of allergies and low white blood cells/platelets should use it with caution. 2. Because this drug can cause allergic reactions, take dexamethasone 20mg 12 hours and 6 hours before administration, and give diphenhydramine 50mg orally and cimetidine 300mg intravenously 30-60 minutes before administration. 3. The commonly used dose of paclitaxel is 135-175mg/m2. The injection solution should be added to 500-1000ml of normal saline or 5% glucose solution. A glass bottle or polyethylene infusion set is required, and a special rubber tube and 0.22μm microporous membrane filtration should be used. 4. After the start of infusion, blood pressure, heart rate, and respiration should be measured every 15 minutes to pay attention to any allergic reactions. 5. Generally, the infusion is 3 hours. 6. After the injection, the blood picture should be checked at least twice a week, and it can be repeated after 3-4 weeks depending on the situation.
Adverse Reactions
1. Allergic reactions: The incidence rate is 39%, of which the incidence rate of severe allergic reactions is 2%. Most of them are type 1 allergic reactions, manifested as bronchospasm, dyspnea, urticaria and hypotension. Almost all reactions occur in the first 10 minutes after medication. 2. Bone marrow suppression: It is the main dose-limiting toxicity, manifested as neutropenia, thrombocytopenia is rare, and generally occurs 8 to 10 days after medication. The incidence rate of severe neutrophilia is 47%, and the incidence rate of severe thrombocytopenia is 5%. Anemia is more common. 3. Neurotoxicity: The incidence rate of peripheral neuropathy is 62%, the most common manifestations are mild numbness and paresthesia, and the incidence rate of severe neurotoxicity is 6%. 4. Cardiovascular toxicity: There may be hypotension and asymptomatic short-term bradycardia. Muscle and joint pain: The incidence rate is 55%, occurring in the joints of the limbs, and the incidence and severity are dose-dependent. 5. Gastrointestinal reactions: The incidence rates of nausea, vomiting, diarrhea and mucositis are 59%, 43% and 39% respectively, generally mild and moderate. 6. Liver toxicity: ALT, AST and AKP increased. 7. Hair loss: The incidence rate is 80%. 8. Local reaction: Inflammation of the infusion vein and local drug extravasation.
