Function and Efficacy

This product is a glucocorticoid drug. It has multiple pharmacological effects such as anti-inflammatory, anti-toxic, anti-shock and immunosuppressive. 1 Anti-inflammatory effect: It has an effect on inflammation caused by various causes except viruses. Glucocorticoids reduce and prevent tissue response to inflammation, thereby reducing the symptoms of inflammation, and can also inhibit the repair of tissues in the late stage of inflammation and reduce sequelae. 2 Immunosuppressive effect: Prevent or inhibit cell-mediated immune responses, delayed allergic reactions, and reduce the expansion of primary immune responses. 3 Anti-toxic and anti-shock effects: Glucocorticoids can improve the body's tolerance, reduce cell damage, and play a role in protecting the body. It also dilates blood vessels, enhances myocardial contractility, and improves microcirculation.

Indication

It is mainly used for replacement therapy of adrenal insufficiency and congenital adrenal hyperplasia.

Usage and Dosage

Oral: For the treatment of adrenocortical insufficiency in adults, the daily dose is 20-30 mg, 2/3 in the morning and 1/3 after lunch. In case of stress, the dose should be increased appropriately, up to 80 mg per day, taken in divided doses. The therapeutic dose for children is 20-25/m2 per day based on body surface area, divided into 3 doses, taken once every hour.

Adverse Reactions

This product generally has no obvious adverse reactions when using physiological dose replacement therapy. Adverse reactions mostly occur when using pharmacological doses, and are closely related to the course of treatment, dosage, type of medication, usage and route of administration. Common adverse reactions are as follows: 1. Long-term use may cause the following side effects: iatrogenic Cushing's syndrome face and posture, weight gain, lower limb edema, purple lines, bleeding tendency, poor wound healing, acne, menstrual disorders, avascular necrosis of the humeral or femoral head, osteoporosis and fractures (including vertebral compression fractures, long bone pathological fractures), muscle weakness, muscle atrophy, hypokalemia syndrome, gastrointestinal irritation (nausea, vomiting), pancreatitis, peptic ulcer or perforation, growth inhibition in children, glaucoma, cataracts, benign intracranial hypertension syndrome, impaired glucose tolerance and aggravation of diabetes. 2. Patients may experience mental symptoms: euphoria, agitation, delirium, restlessness, disorientation, and may also manifest as inhibition. Psychiatric symptoms are particularly prone to occur in people with chronic wasting diseases and those who have had mental disorders in the past. 3. Concurrent infection is the main adverse reaction of adrenocortical hormones. Fungi, tuberculosis, Staphylococcus, Proteus, Pseudomonas aeruginosa and various herpes viruses are the main ones. 4. Glucocorticoid withdrawal syndrome: Sometimes patients experience dizziness, fainting tendency, abdominal pain or back pain, low fever, loss of appetite, nausea, vomiting, muscle or joint pain, headache, fatigue, and weakness after stopping the drug. If careful examination can rule out adrenocortical insufficiency and recurrence of the original disease, it can be considered as a glucocorticoid dependence syndrome.

Precautions

Patients who are allergic to this product and other steroid hormones are prohibited from using it. Patients with the following diseases should not use it in general. In special cases, the pros and cons should be weighed, but attention should be paid to the possibility of worsening of the condition: severe mental illness (past or present) and epilepsy, active peptic ulcer disease, recent gastrointestinal anastomosis, fractures, trauma repair period, corneal ulcer, hyperadrenocortical syndrome, hypertension, diabetes, pregnant women, fungal infections that cannot be controlled by antibiotics, chickenpox, measles, severe osteoporosis, etc.

Special Population Medication

Precautions for children: Children should be very cautious when using adrenocortical hormones for a long time. Precautions for pregnancy and lactation: 1. Medication during pregnancy: Glucocorticoids can pass through the placenta. Animal experimental studies have confirmed that administration during pregnancy can increase the incidence of cleft palate, placental insufficiency, spontaneous abortion and intrauterine growth retardation. The use of pharmacological doses of glucocorticoids in humans can increase the incidence of placental insufficiency, neonatal weight loss or stillbirth. 2. Medication during lactation: Since glucocorticoids can be excreted in breast milk, they can cause adverse effects on infants, such as growth inhibition and adrenal cortex function inhibition. Pregnant and lactating women should avoid using them as much as possible after weighing the pros and cons. Precautions for the elderly: Elderly patients are prone to hypertension and diabetes when using glucocorticoids. Elderly patients, especially postmenopausal women, are prone to aggravating osteoporosis when using glucocorticoids.

Drug Interactions

1. Non-steroidal anti-inflammatory analgesics can enhance the effect of this product on gastrointestinal ulcers. 2. It can enhance the hepatotoxicity of acetaminophen. 3. When used in combination with amphotericin B or carbonic anhydrase inhibitors, it can aggravate hypokalemia. Long-term use with carbonic anhydrase inhibitors can easily lead to hypocalcemia and osteoporosis. 4. When used in combination with protein anabolic hormones, it can increase the incidence of edema and aggravate acne. 5. Long-term use with anticholinergics (such as atropine) can cause increased intraocular pressure. 6. Tricyclic antidepressants can aggravate the mental symptoms caused by this product. 7. When used in combination with hypoglycemic drugs such as insulin, the dosage of hypoglycemic drugs should be appropriately adjusted because this product can increase blood sugar in diabetic patients. 8. Thyroid hormones can increase the metabolic clearance rate of this product, so when used in combination with thyroid hormones or antithyroid drugs, the dosage of the latter should be appropriately adjusted. 9. When used in combination with contraceptives or estrogen preparations, the therapeutic effects and adverse reactions of this product can be enhanced. 10 When used in combination with cardiac glycosides, it can increase the toxicity of digitalis and the occurrence of arrhythmias. 11 When used in combination with potassium-excreting diuretics, it can cause severe hypokalemia and weaken the sodium-excreting and diuretic effects of diuretics due to water and sodium retention. 12 When used in combination with ephedrine, its metabolic clearance can be enhanced. 13 When used in combination with immunosuppressants, it can increase the risk of infection and may induce lymphoma or other lymphoproliferative diseases. 14 It can increase the metabolism and excretion of isoniazid in the liver, and reduce the blood concentration and efficacy of isoniazid. 15 It can promote the metabolism of mexiletine in the body and reduce the blood concentration. 16 When used in combination with salicylates, it can reduce the concentration of plasma salicylate. 17 When used in combination with growth hormone, it can inhibit the latter's growth-promoting effect.
