Appearance
This product is a capsule, the contents are white or off-white powder, odorless.
Indication
A whole-gastrointestinal prokinetic drug. It is mainly used for functional dyspepsia, upper gastrointestinal discomfort with negative X-ray and endoscopy examinations, with symptoms such as early satiety, fullness after meals, decreased appetite, bloating, excessive belching, lack of appetite, nausea, vomiting or ulcer-like complaints (burning pain in the upper abdomen). It can also be used to treat mild reflux esophagitis.
Usage and Dosage
Oral treatment: Dosage: The maximum daily dose is 30 mg. Adults: Depending on the severity of the disease, the total daily dose is 15-30 mg, divided into 2-3 doses, 5 mg each time (the dose can be doubled). Children weighing 25-50 kg: The maximum dose is 5 mg each time, four times a day. Or as directed by a doctor. Children weighing less than 25 kg: 0.2 mg/kg body weight each time, three to four times a day. Or as directed by a doctor. It is recommended to avoid taking it with grapefruit juice as much as possible. In case of renal insufficiency, it is recommended to reduce the daily dose by half.
Adverse Reactions
Due to the pharmacological activity of this product, transient abdominal cramping, borborygmus, and diarrhea may occur. If abdominal cramping occurs, the dose may be halved. Occasionally, allergic reactions including rash, itching, urticaria, bronchospasm, mild transient headache or dizziness, and dose-related urinary frequency have been reported. Very rare reports of arrhythmias, including ventricular tachycardia, ventricular fibrillation, torsade de pointes, and QT prolongation. Most of these patients often take several other drugs at the same time - including drugs that inhibit the CYP3A4 enzyme, or have heart disease and risk factors for arrhythmias. Precautions
Patients with known allergies to this product are contraindicated. Concomitant oral or parenteral use of drugs that strongly inhibit CYP3A4 enzymes is prohibited, including: triazole antifungal drugs; macrolide antibiotics; HIV protease inhibitors; nefazodone; (see Drug Interactions). Patients with heart disease, arrhythmia, QT interval prolongation are contraindicated, and it is prohibited to use it with drugs that cause QT interval prolongation; patients with water and electrolyte disorders, especially those with hypokalemia or hypomagnesemia, are contraindicated; patients with bradycardia are contraindicated; patients with congenital QT interval prolongation or a family history of congenital QT interval prolongation syndrome are contraindicated; patients with pulmonary, liver, and kidney dysfunction are contraindicated; infants and young children are contraindicated.
Special Population Medication
Precautions for children: Infants and young children are prohibited. Precautions for pregnancy and lactation: Although it does not affect embryo formation in animals and has no original embryo toxicity. It also has no teratogenic effect, but if it is used during pregnancy, especially in the first three months of pregnancy, the pros and cons should be weighed. Although the amount excreted through breast milk is very small, it is still recommended that breastfeeding mothers do not use it. Precautions for the elderly: In the elderly, due to the moderate extension of the elimination half-life, the steady-state plasma concentration will generally increase, so the therapeutic dose should be reduced as appropriate.
Drug Interactions
The main metabolic pathway of this product is through CYP3A4 enzyme metabolism. If the drug that can inhibit this enzyme is taken orally or parenterally at the same time, it may lead to an increase in plasma cisapride concentration, thereby increasing the QT interval and the risk of arrhythmia, including ventricular tachycardia, ventricular fibrillation and torsade de pointes. Therefore, it is forbidden to take it at the same time with these drugs.
